Brief Answer:
Cause of fall and bleed is vital to be diagnosed.
Detailed Answer:
Hello Sir,
Thank you for the follow-up and clarifications.
I've seen the MRI report.
The MRI report shows evidence of injury to the brain. There is bleeding within the brain on the right side. Possibly due to the fall
The areas of the bleed can affect facial muscles (right side) as well as those of the arm and leg (on the left side). As the bleeding is not applying too much pressure on his brain, there is nothing urgent to worry about as of now.
However, it is possible for the bleeding to increase (usually within 2 weeks of initial injury) and for there to be complications such as facial muscle weakness (slurred speech, unable to chew food properly leading to coughing or choking on food & difficulty swallowing), weakness in muscles of arms and legs on the left side (tripping and falling often, difficulty in changing positions) and seizures (due to scarring or brain tissue).
It is also vital to detect cause of fall and bleed (blood pressure fluctuations, heart rhythm disturbances or other abnormalities, coagulation abnormalities ).
*has he consulted with any neurologist and/ physician?
*has he undergone any other tests? After and before the fall?
*are his symptoms (giddiness, headache, unclear words) getting better or worse or is the same?
*did he have all these symptoms only after the fall?
*did his leg pain start after the fall? Where did he fall?
*which side of his head does it hurt and which side got hit?
*does he smoke or consume alcohol?
*what's his height and weight?
*what is his profession?
* did his doctor say he had any muscle weakness?
*was he alone when he fell? did he recover and stand up or had to be woken up by someone?
I'd recommend:
- a physician and neurologist consult.
- blood tests (complete blood count, liver & kidney function tests, lipid profile, coagulation profile, blood glucose panel)
- urine analysis
- electrocardiogram and echocardiogram.
- carotid doppler.
- complete and thorough neurological exam and regular monitoring.
- he should avoid driving and traveling alone anywhere for the next two months.
- repeat CT scan in 2 weeks to check for resolution of the bleed.
- anti-seizure medications.
He may not need all of these tests at a time or may need more. This will be advised based on the clinical findings and outcome.
It is vital to rule out other possible causes of the fall and bleed.
If he develops worsening of present symptoms, or develops new muscle weakness or is drowsy, has difficulty breathing, please visit the emergency right away.
Hope this helps and I've answered your query.
Please let me know if there is anything else I can help you with.
Wish him a good health.
Regards,
Dr. Shuba Hariprasad

Brief Answer:
I am concerned about why he fell.
Detailed Answer:
Hello,
Thank you for the clarifications.
As he is feeling better, there seems to be nothing to worry about at present, regarding the consequences of the injury.
However, I am concerned about why he fell. You mentioned that he had low blood pressure. How was it when he checked it out after the fall?
I recommend that he sees a physician to make sure other parameters (as mentioned in my previous reply) such as coagulation disturbances, blood pressure and heart be checked out to make sure everything is alright.
If all is OK, then just close monitoring for 8 weeks is advisable. Physiotherapy, speech therapy should also be continued.
Hope this helps. Please let me know if there is anything else I can help you with.
Wish you all good health.

Dr. Shuba Hariprasad
