What Does This MRI Report Of Brain Indicate?

Question:

My recent MRI Brain report as follows.
Please explain what this means ?
Report :
The study reveals evidence of few focal Fast FlLAIR hyperintensities in bilateral periventricular white matter suggestive of lacunar infarcts
Opinion: MR findings are suggestive of a few lacunar infarcts in supratentorial brain paranchyma.No other significant intracranial lesion is seen.Please correlate clinically.

Do you think above MRI findings are suggestive of any underlying neurological problem? as I am a chronic depressive patient on medication with very slow improvement(since almost 20 years with cognitive problems like short term memory, constant low mood and severe fatigue etc ) I can send the scanned copy of MRI Brain report also, if required.Currently on Duloxetine 20mg in morning and Flunil 60mg after lunch.Also I experience disturbed sleep at night,which only my partner observes( like severe shaking of legs and arms in sleep,loud and incoherent nonsense talking while in sleep, sometimes I fall out of bed but soon I realize ), this is more pronounced when I miss/forget my dose of antidepressant medication.Kindly suggest any suitable medication also.I am attaching my ECG and EEG report also

I have noted your symptoms and MRI brain reports.

The MRI is suggestive of reduced blood flow to small areas in brain. Also, your symptoms are mostly related to depression. However, memory impairment may occur due to lacunar infarcts (less blood flow to brain).

We need to evaluate causes of reduced blood flow to brain, such as testing for sugar, cholesterol, homocysteine in blood.

Thanks a lot for your excellent and very relevant reply, I m sure there is really lack of blood flow to brain. Infact I experienced severe brain related symptoms during first attack like complete numbness of brain, brain fog, difficulty in walking and balance ,severe memory impairment , severe fatigue and constant low/dull mood /complete blankness with no emotions/body pains etc. Doctor put me on Flunil 40 mg , but I could never come back to my original sharpness in memory energy etc even after using antidepressant medication for more than 20 years. I used to have excellent memory and used to topper throughout my academic career,and good energy levels before the sudden first severe depressive attack.I recently got my Sugar and Cholestrol level checked. The results are as follows
Blood sugar Random -89 mg/dl, HbA1C -5.6%, Cholestrol HDL -44 mg/DL Range (40-60)cholesterol LDL -107.6mg/dl - Range (0-100) .I did not get homocysteine in blood got checked. Please let me know shall I get it checked, if so please write exactly the parameters to be get it checked and also can you suggest any good medication to increase blood flow to brain as I m facing cognition ,fog ,fatigue related issues of brain since more than 20 years.Also any medication to my sleep related issues I mentioned in my earlier description since I started using depression medication.

1. At this stage, you can take piracetam (nootropil) tablets, which are helpful in improving blood circulation to brain, as well as memory.

2. The blood test is called: Fasting serum HOMOCYSTEINE

3. Regarding the sleep disorder, you could be having PLMD- periodic limb movement disorder. If confirmed, there are good medications to treat that.

Dear Sir
Thanks a lot for your prompt and precious advise.I am a XXXXXXX Government officer in Central Govt Ministry at New XXXXXXX The quality of life very much deteriorated ( severe fatigue/cognition issues/lack of motivation etc) excatly opposite to my original self and outlook after my post sudden depressive episode with no obvious reason happened as a student when I was 22 years old.

What is the daily dosage of piracetam to be taken ? how many times (morning or evening with food or without food) and how long I need to take this medication?
Please name confirmatory tests i can get it done,regarding exploring possibility of PLMD.

1. I usually start Piracetam at a dose of 800 mg two times daily for my patients, after food in morning and night. The dose can be increased to 1200 mg two times daily, if needed. It is sold in the name of Nootropil.

2. For PLMD, the test is called polysomnography (or sleep studies), available in most standard hospitals.
