
What Does This MRI Report Of Right Shoulder Indicate?

I have been in severe pain for months (especially difficult just trying to sleep). Intense physical therapy and medication have done little in my case. Is surgery needed?

According to the MRI Report you do not have any serious conditions like Rotator cuff tear or Biceps tendon disorders which usually cause pain.
You just have mild arthritis of the acromio-clavicular joint of the right shoulder which may be the cause for the pain you experience. This can be treated by oral medication, anaesthetic and steroid injection into the joint.
A steroid shot will give you considerable relief from the symptoms of arthritis for a period of upto 3 months. Some studies demonstrate upto 5 months relief with Platelet rich plasma which can be derived from a sample of your own blood and has no side effects. Apply ice to the painful area and avoid overhead activities and carrying backpacks or lifting weights. Also you must get tested for Diabetes to rule out causes of Frozen shoulder.
I would recommend you consult your Sports Medicine specialist to get examined for any Acromio-clavicular joint instability. If there is significant instability then surgery may be required otherwise surgery is not indicated in your problem.

Hi there.
The Steroid shot should be infected only in the Acromioclavicular joint as there is arthritis noted only in this joint. It should give your significant relief and very negligible side effects because of no intervening muscles or tendons.
The PRP therapy is part of blood and blood products which is not related by the FDA. The FDA has mentioned that PRP can be used by the Doctor in injections based on his clear clinical judgement of the need to object versus the risk profile which needs to be clearly understood.
I have not come across any side effects of PRP objective in my practice of date.
Kindly consult your Doctor regarding the site of the injection which was administered, if it was into your shoulder joint or in the Acromioclavicular joint. Discuss his views about PRP and it's effects as well.
