Brief Answer:
CABG surgery if preferred over PTCA angioplasty if patient fit for surgery

Detailed Answer:
Hi XXXXXXX
Thanks for writing in to us.

I have read through your query in detail.
Please find my observations below.

1. The indication of coronary artery bypass graft CABG surgery includes the following from the recommendations of XXXXXXX College of Cardiology (ACC) and the XXXXXXX Heart Association (AHA) as quoted below

(i) Left main coronary artery stenosis greater than 50%
(ii) Stenosis of proximal left anterior descending LAD and proximal circumflex more than 70%
(iii) Triple vessel disease in asymptomatic patients or those with mild or stable angina
(iv) Triple vessel disease with proximal LAD stenosis in patients with poor left ventricular function
(v) One or Two vessel disease and a large area of viable myocardium in high risk area in patients with stable angina
(vi) More than 70% proximal left anterior descending LAD stenosis with either ejection fraction less than 50% or demonstrable ischemia on noninvasive testing
(vii) Disabling angina
(viii) Ongoing ischemia in the setting of a non ST segment elevation myocardial infarction that is unresponsive to medical therapy
(ix) Poor left ventricular function but with viable, nonfunctioning myocardium above the anatomic defect that can be revascularized

2. Your relative clearly has indications which include points from the above list.

3. Percutaneous transluminal coronary angioplasty (PTCA) is suggested if the patient is not fit for surgery or if there is emergency situation.

Hope your query is answered.
