Question: hi. I am 43years old and just done a blood test. Hb 5.5, PCV 22, MCV 69, MCH 18, MCHC 26. My grandmother just had a stroke and pass away a month ago and I was very tired.

Is my condition critical or perhaps my health result is partly affected by recent family problem?

Thanks

Brief Answer:
Your condition is serious, requires detailed investigations and treatment

Detailed Answer:
Hi XXXXXX,
Thanks for writing in to us.

I have read through your query in detail.
From your test results it appears that you have severe anemia. This is a condition when the hemoglobin content in blood is below normal. It should be above 11 grams/ dl and you have less than half the normal levels.

Similarly PCV normal is 34 percent and you have 22 percent.
MCV normal is 75 and you have 69
MCH normal is 26 and you have 18
MCHC normal is 32.5 and you have 26

It is important to know the type of anemia and its cause. Doing a peripheral smear is required and also you must get a complete general clinical examination done.

In females at your age anemia can be due to increased blood loss due to menstrual periods or though vomiting blood or blood in stools. Anemia can also occur due to nutritional deficiency or any disease condition that is limiting the generation of red blood cells.

Your condition might be considered serious and after consultation with doctors, it might be necessary to do blood transfusion after proper cross matching. Taking iron supplements and Vitamin B12 is not enough. It is important to eat good nutritious food with high proteins.

Coming to the point raised by you, the death of your grandmother is unlikely to be responsible unless you have not been eating properly for weeks due to your family problems.

Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.

Brief Answer:
Please get general health check up done

Detailed Answer:
Hi XXXXXXX,
Thanks for writing back with an update.

Flu usually improves over a week or maximum 10 days. If you have been having cough for more than two months with significant loss of weight and fever then the first condition that needs to be ruled out is Tuberculosis. I am saying this because TB is the commonest infection which can cause coughing for weeks in XXXXXXX

It is not necessary that you have got TB but severe anemia weakens the immune system and can cause infections easily. Please do not worry about it and get checked completely and know if you have any other condition that is responsible for your anemia.

Getting a Chest X ray for cough and Ultrasound scan abdomen as a routine check up is recommended.

Hope your query is answered.

Brief Answer:
You must get medical check up done at the earliest

Detailed Answer:
Hi XXXXXX,
Thanks for writing back with an update.

My prayers are with you and may God bless the departed soul.
It is possible that you are a strong person mentally and have done a lot during the time of your family crisis. Life has to continue and you should take care of yourself.

If you were my patient, I would have requested you to get evaluated immediately and take necessary medications and reduce stress as much possible.

Its nice to know that you have a good appetite, sleep well and doing swimming. Sometimes patients with hemoglobin of 5.5 grams/ dl can collapse due to exhaustion and I dont want you to take risks on your life. Please remember that life is a precious gift and you must take care of yourself.

If you still miss your grandmother and wish to express your feelings to another person then please do not hesitate to talk to a clinical psychologist and know ways to come out of grief.

Hope your query is answered.
Do write back if you have any doubts.

Regards,
Dr.Vivek
