What Does This Blood Report Indicate?
I have irregular sleep, palpitation at times, anxiety, cramps in leg, mood swings, high acidity, exhausted at night.

I have gone through your query. From your symptoms and Blood test report you are most likely having iron deficiency anemia and vitamin D insufficiency. Alkaline phosphatase is increased slightly which also can be due to vitamin D insufficiency. Vitamin B12 is also going towards lower limit.
Substitution of these will be helpful to clear up this. Taking oral supplements of Iron , vitamin B12 and folic acid will be helpful for iron deficiency anemia. You should also take vitamin D supplements. Initially can take 1000 IU per day or 8000 IU once in 8 days for 3 to 6 months. After that you can take recommended daily dose 600 IU per day.
Your symptoms will be improved once you start therapy.

For prescription you can consult your GP. If you were my patient then i would advise brands like DEXORANGE syrup (ferric ammonium citrate 160 mg, cyanocobalamin 7.5 mcg, folic acid 0.5 mg/15 mL) 15 ml twice daily. Therapy should be 3 to 6 months with a follow up in 3 and 6 month with blood check up.
Vitamin D3 i would suggest softgel caps containing 1000 IU to take once daily. It is available in brands like Nutri d3 , Wal d3 , Uprise d3 etc. This also is for 3 to 6 months with follow up.
