What Does This Blood Report Indicate?

Posted on Mon, 24 Oct 2016
Question: My husband has not been feeling well, off and on, for over a year now. He changed his diet dramatically over 6 months ago to mostly whole healthy foods, fruits and vegetables and little meat and alcohol. Though he feels much better, he still feels unwell. Recently his blood result test came back with this:

RBC of 5.3

Hematocrit of 51.4

Platelets of 327

Hemoglobin of 16.8

LDL Cholesterol 135

Deficient in Vitamin D, 25-Hydroxy, level at 22.6

He doesn't fit the total profile of Polycythemia. He also has never had heart issues and no genetic heart issues either. Do these blood results indicate something that we are not seeing?

Thank you.

Detailed Answer:
Hello!

Welcome and thank you for asking on HCM!

I carefully reviewed all your husband blood lab tests and would explain as follows:

1- His hemoglobin levels and Hct are a little elevated, which indicates policitemia.
These levels are quite borderline. They could be related to a secondary cause like lung disease, smoking contacts, sleep apnea, living in high altitude, etc..
Does your husband suffer from any lung disease ? Do you live in high altitude?

2- His cholesterol levels seem to be a little elevated. But his total cholesterol/HDL ratio is within the normal ranges, which means that he does not need any treatment for his purpose, as he doesn't suffer from dislipidemia.

3- His vitamin D levels are low, which means that he needs to take vitamin D supplements. Chronic fatigue can be related to vitamin D levels. This vitamin is usually produced in our skin, during the sun contact. Some supplements would be helpful.

I would also recommend performing some other blood lab tests to investigate for possible secondary causes of elevated hemoglobin and Hct levels:

- a chest X ray study
- pulmonary function tests
- blood gas analysis
- a cardiac ultrasound to examine his heart function and structure.

You should discuss with his doctor on the above issues.

Hope to have been helpful!

Feel free to ask me again whenever you need!

Kind regards,

Dr. Iliri

Hello!

Thank you for the additional information!

Coming to this point, I would recommend performing inflammation tests (PCR, sedimentation rate ) and allergy tests.

Psoriasis is a chronic inflammatory disease, which can cause join pain.

From the other hand, his symptoms ( itching, discomfort after eating ) could be related to an allergic reaction.

I would also recommend starting vitamin D supplements and performing pulmonary function tests and blood gas analysis to investigate for a possible lung disease .

You should discuss with your doctor on the above issues!

Best wishes,

Dr. Iliri
