Question: Hi Dr,
I'm XXXX and I have done kidney transplant on 28th Sep, all the reports post transplant shows OK /improving except TLC and DLC which is increasing day by day as below (latest) Creatinine fluctuates between 1.6 to 1.84 and the Urea on the higher side (39, 40, 43). Pls advice

Neutrophils 82.9%
Lymphocytes 10.0%
Monocytes 5.9%
Eosinophils 0.9%
Basophil 0.3%
Platelet count 409000

RDW CV 21.3%

Thanks for query .
Based on the facts that you have postedyou haave undergone surgery of Rnal Transplant very recently 2 weeks back and your routine post operative check up has revealed raised WBC count .
.
Raising of the WBC count suggests infection and is not uncommon after renal transplant because of poor resistance due to renal failure ,raised creatinine etc .However it is nothing to be worried about. The problem will get resolved within a month by taking appropriate antibiotics ,multi vitamins and other supportive measures .

Thanks for the detailed note. How can we figure out the type of infection and medication accordingly. Should I do any tests?. My Dr asked me to do Dopper, urine routine and 'culture and those are normal. Raised WBC with high Creatinine keeping me worried. no medication advised yet than increasing fluid intake. The ESR also showing high (25mm/1st hour) Pls advise

If you have fever get blood culture done

Hello
Thanks for follow up .
Raised WBC count is reaction of your immune system to major surgical trauma .If urine culture is negative and if you have fever with raised WBC count may do Blood culture .

Do not expect your creatinine to come to normal values in immediate post operative period A creatinine of 1.6 to 1.8 is in well accepted range .

WBC count will also come down have a patience
