Question: I am 26 years old unmarried female. having regular periods
Have below test results:
FSH -17
AMH - 1 ng/ml
TSH - 10
ovarian volume - right ovary - 6.7 cc and left - 9.8 cc
I was wondering even with good ovarian volume how come I have low AMH levels?
Also I read couple of places TSH might impact high FSh and some places they say no relation. Is TSH affecting FSH? and why do I have low AMH even after having good ovarian volume and size?

Brief Answer:
About low AMH level.

Detailed Answer:
Hello,
I can understand your concern regarding to ovarian reserves and your age.
Low AMH level and high FSH are indicative for a primary ovarian pathology. The normal ovarian volume is due to normal quantitative eggs but probably the quality is effected and this may be caused by genetic mutations.
As the FSH correlates with low AMH, it is indicative for ovarian cause. There are studies where a correlation between TSH and FSH is found but i do not think is your case.
Anyway you should start treatment with levothyroxine and recheck the hormones after 4-6 weeks.
Hope the information will help.
Kind regards,
Dr.Mirjeta

Thanks Doctor. But I always read AMH indicates quantity, if my egg quantity is good, why my AMH is low? also I have good antral follicle count. Im deficit of vitamin D, I have levels of 19 which indicates moderately deficit. I have started to practice yoga, have gone for a healthy diet. Thinking of going for acupuncture as well. Do you think these will help? Can I get conceived after I get married? I'm a healthy woman with normal BMI

Brief Answer:
Start levothyroxine.

Detailed Answer:
Hi again,

If the ultrasound shows good antral follicle count and you have normal menstrual cycle than FSH 17 IU/L is considered normal if you have been in the ovulation phase. The normal range of AMH is quite wide, even in this age group, so all you have to do is to correct the TSH level and not to gel worried.

Wish you good health,

Kind regards,
Dr.Mirjeta

Hi Doctor, FSH was done on day 4 of my cycle. it was not during ovulation phase. can I reduce it by going to acupuncture and having good diet and regularly performing yoga?

Some more info.

Detailed Answer:
Hi again,
Before giving some more suggestions, could you please tell me why did you check FSH, ovarian volume and AMH, as your menstrual cycle was XXXXXXX and you are unmarried?
It is important and helps a lot in the interpretation of the results in your case.

Dr.Mirjeta

I had irregular periods earlier, i used to have 2 months once. from past 3 years it's normal every 28 days. and recently once I missed my periods, that's when doctor asked me to get all these tests done. but I missed only a month and again it became normal without any medication.

Brief Answer:
Start levothyroxine.

Detailed Answer:
Hi again,
In this context the FSH and AMH would be interpreted within normal ranges.
Anyway, i would suggest to start levothyroxine and check the TSH level after 6 weeks.
Best wishes,
Dr.Mirjeta
