What Does This Blood Report Indicate?
I'm gathering medical records from the past two months I have been ill. This started with heart palpitations, vision impairment, and headache. It has progressed into nerve pain/burning and tingling (which subsided), choreaform type movements, tremors, vertigo, low blood pressure, weight loss, and most recently abdominal pain starting in the upper left quadrant and showing up in the mid abdomen and occasionally lower left and right quadrants. I feel nauseous at times, but still hungry. When I eat, my stomach feels immediately sick and I get pain in my upper back as well. I have not been able to go to the bathroom well for about five days. The pain is also in my pelvic area and lower back spreading to groin and leg area.
My test results show a rising WBC
Elevated MPV
Low Creatinine
Elevated Bilirubin
Low AST
Low ALP
What would be the reason for these results?
Supportive care , needs work up and management.
Detailed Answer:
Thank you for asking
I read your question and i understand your concern. Lets first discuss the labs you shared. Elevated MPV ( mean platelet volume ) is mostly reactive thrombocytosis and not a concern, could happen even in a benign infection like flu and cold. Creatinine are desired low so are AST and ALT and considered normal. Elevated bilirubin needs to be sorted out how elevated, if its 2 or around 2 then its not that elevated.
Now your symptoms , Palpitation, headache, vision impairment, abdominal pain followed by diet radiating in all the quadrants needs a little clinical correlation. Get a visual acuity test to rule out any eye sight problem, Tremors need a neurological consult but i believe its likely a benign essential tremor common in anxiety and panic attacks and your other symptoms of palpitations and headaches and bowel symptoms fit the description. Abdominal symptoms look like irritable bowel syndrome and will need diet and lifestyle adjustments. You should eat small frequent meals, avoid triggers like spices, processed foods, canned foods, soda, beverages, alcohol, tea, coffee, chocolates etc. Use yogurt with peppermint, more fiber, fresh fruits and vegetables and see if it helps.
Consult your doctor and let them clinically correlate all your symptoms for you. I hope it helps. take good care of yourself and dont forget to close the discussion please.

My bilirubin levels have fluctuated from between 1.8 and 3.6 with the most recent level at 2.3. Sometimes I am jaundiced. I saw an opthmologist to rule out vision problems and he said my eyes are actually great and he believes it is neurological. I have the abdominal symptoms regardless of what I eat, and they came on suddenly approximately a week ago. I am already on a somewhat restricted diet, so I don't believe I ate anything to trigger it.
Thank you again.
Neurological examination needed.

Thank you for getting back to me
its good to hear that bilirubin levels are not that high, up to 2.3 is acceptable and nothing to worry about. Abdominal pain on restricted diet should help. Take some antispasmodics and follow diet lifestyle recommendation mentioned above and see if it helps. Do get a neurological exam, as it could be migraine variant and needs to be evaluated for. Hope for the best, i am sure supportive remedies will relive the symptoms.
