default
Posted on Mon, 14 Dec 2015
Question: can you please assist with the possible cause resulting from a chest xray....consolidation appears in the left lower lobe...At the same time my mother started to breath differently like she has a cold or she is restricted in her breathing..also is it possible or likely that it is a small pulmonary embolus...the radiologist is very very young.

Brief Answer:
Yes, Pulmonary embolism and infarction look like consolidation on x ray.

Detailed Answer:
Thanks for your question on Health Care Magic.
I can understand your concern.
Chest x ray is only screening tool.
It is not 100% confirmatory for consolidation.
Actually Pulmonary embolism and infarction look similar to consolidation on chest x ray.
Consolidation is due to lung infection. And hence fever should be present if it is pneumonia and consolidation.
Breathlessness or breathing difficulty is seen in consolidation but it is not common.
Breathing difficulty is common in Pulmonary embolism and infarction.
Actually CT Pulmonary Angiography is needed to rule out Pulmonary embolism and infarction.
So please let me know
1. Is she having fever?
2. Is she having cough and expectoration?
3. Since how many days she is having these symptoms?
Please reply me answers of above asked questions, so that I can guide you better.
I will be happy to help you further.
Wish you good health. Thanks.

Brief Answer:
Can you send me x ray image?

Detailed Answer:
Thanks for your follow up question on HCM.
Pneumonia is unlikely because cough and expectoration are common symptoms of pneumonia.
Since these symptoms are absent, possibility of pneumonia is low in her case.
Can you please send me x ray image?
Is she taking any drugs at present?

Brief Answer:
No need to worry much for Pulmonary embolism or consolidation.

Detailed Answer:
Thanks for your follow up question on HCM.
Now I can see the images you have attached.
I looked at very carefully. In my opinion, you should not worry much about Pulmonary embolism or consolidation.
She is having prominent bronchovascular markings in lower zones and emphysematous lungs.
And so possibility of bronchitis is more in her case.
And hence she needs PFT (Pulmonary Function Test) for the confirmation.
Please let me know
1. Has she ever undergone PFT ()?
2. Is she having breathing difficulty for the first time or it is recurrent?
Please reply me answers of above asked questions, so that I can guide you better.
I will be happy to help you further.

Brief Answer:
Is she smoke?

Detailed Answer:
Thanks for your follow up question on HCM.
I can understand your concern.
Looking the chest x ray possibility of consolidation is minimal. And possibility of Pulmonary embolism and infarction is also less because Pulmonary infarction causes wedge shaped opacity and it is not seen is her chest x ray.
Her chest x ray is more suggestive of bronchitis.
Please let me know
1. Is she smoke?
2. Is she having exposure to passive smoking?
3. Is she having exposure to chemicals?
4. Has she ever undergone PFT?

No need to worry much since she is not having any risk factor.
Between I am not getting you.
What is CAL? And why you are so much confident about PE? Did you get done Pulmonary Angiography?

Pt 11...that ok for a fit healthy XXXXXXX person..but not for immobile...any way just thinking outside the square or worst case.thanks Dr..

Answered by Dr. Drkaushal85 (11 minutes later)
Brief Answer:
Get done PFT.

Detailed Answer:
Thanks for your follow up question on HCM.
I can understand your concern.
No need to worry too much. She is not having consolidation or PE (Pulmonary embolism).
She is mostly having bronchitis.
And chronic airflow limitation is seen in bronchitis.
And for diagnosis of bronchitis, she needs to get done PFT.
PFT will also tell you about severity of the disease and treatment of bronchitis is based on severity only.
She may need inhaled bronchodilators and inhaled corticosteroid (ICS).
Don't worry, she will be alright. Consult pulmonologist and discuss all these. She will definitely improve with inhaled treatment.
Hope I have solved your query.
