What Does This Chest X-ray Report Indicate?
Question: Hello Doctor ,
hv got diagnosed my mother , attaching report of chest x-ray PA view& CBC /RBS/HBA1c
X-ray report is given below:X-RAY CHEST PA VIEW
Soft tissues and bony cage under view are normal.
Lung fields show non-homogenous opacity in right lower zone.
An ill-defined opacity is also seen in right upper zone.
Two nodular lesions are seen in left middle zone.
Hila and mediastinum are normal.
Both CP angles are blunted.
Cardiac size is increased.
CT ratio is 14.1 : 26.6.
IMPRESSION : Pneumonitis, cardiomegaly and bilateral pleurisy with
nodular lesions left mid zone.
Advised : CECT Chest and comparison with previous films if any.

The x ray report is more suggestive of pneumonia (lung infection).
But possibility of congestive cardiac failure ( CCF ) is also there, because of
1. Age (69 years)
2. Cardiomegaly on chest x ray.
3. Bilateral pleural effusion.
So we need to rule out pneumonia and CCF.
CECT thorax is needed to rule out lung infection.
2d echo is needed to rule out CCF.
Nodular opacities in lung at her age can be seen in malignancy. CECT thorax will also rule out malignancy.
So get done both CECT thorax and 2d echo.

Is there any reason of worry , if unfortunately all mentioned follow-up testes are positive , are they curable by medicine /surgey , sorry for being negative .

So possibility of pneumonia and CCF is high in her case.
Diabetics are always at great risk for lung infection. So pneumonia is one possibility.
Same way, diabetics are also at great risk of CCF. So get done CECT thorax and 2d echo to rule out above mentioned conditions.
Better to admit her in hospital as she may need insulin injections to control diabetes.
