Dear Sir
An ECG is best interpreted in light of clinical information. With this comment, I have following input about the ECG uploaded.
1. The ECG shows Normal sinus rhythm, right bundle branch conduction delay, J point elevation in V1-3 and rightward axis of P waves. Though all these feature points towards a lung pathology as a possible cause of his breathlessness, clinical correlation is a must. Dilligent chest examination, X-ray chest, PFT and echocardiogram should be done. Respiratory rate, use of accessory muscles is important in clinical examination.
2. If there is a history of sudden death in family or resuscitated cardiac arrest in this patient or in family, I would seek an electrophysiologist opinion in the case. I would also seek it if the ECG pattern does not disappear after the current febrile illness is over and the patient is relatively young. This is important as the pattern in given ECG may confirm to Brugada's syndrome. Though it does not appear classical in this ECG.
