Question: Hi i was taking bp medicines from 2014 .metolar xr 100 twice daily, thelma ct forte once daily, cilacar 5mg, atarax and pantacid once daily. My 2014 test reports were all perfect. And in these three years my bp was also perfect and i reduced 20kgs with diet , exercise and walk and i never touched sweet things because i dont like sweets. But i was shocked to see my current test report i have fasting suger 368, sodium is low 131, glucose in urine is 4plus (2.0g/dl),hba1c is 13.3,cholestrol total 202, triglycerides 211,hdl 39,ldl 121, vldl 42.20, c reactive protin serum 6.80, chloride is low 94.
When i went to dr. Yesterday ,He increased my bp medicine cilacar 10 instead of 5,
Ecosparin AV 75 once for 20 days, glyciphage G1 twice daily.
My question is how could it be possible to get these complecations inspite of 20 kg weight loss, healthy eating and avoiding sweets. Is it due to my bp medicines side effect?. And do i have a chance of survival with so much problems. We dont have a family history of sugar. And i m really hopless. Plz guide .

Brief Answer:
Their are other causes of diabetes

Detailed Answer:
Hi,
Welcome.
This is Dr Sameer, cardiologist from Fortis hospital.
First i would like to break your myth about diabetes. It is not caused because of excess intake of sweets. Diabetes is actually a autoimmune disease with multifactorial causes. Actually our pancreas which forms insulin(to metabolise glucose) gets attacked by some chemicals which are formed in our body against it(antibody). This causes decline in function of pancreas & thus decreased or defective insulin formation & thus Diabetes.
Hypertension is completed different from Diabetes & hypertension does not cause diabetes.
Coming now to control & future risks.
You have very high HbA1c (which tells glucose control for 3 months). This means the diabetes has been there for some time but not detected.
Complications in diabetes can be involvement of eye(retina), kidney or nerves & long term uncontrolled glucose can effect heart also.
I will recommend you for certain tests - Kidney function test, evaluation of retina by a ophthalmologist & a TMT examination.
If diabetes remains in control & complications does not develope than you need not worry & just continue taking medication & keep dietary restrictions.
Hope this answer helps.
Thanks & take care

Brief Answer:
Diet as advised ahead

Detailed Answer:
Hi XXXXXXX
Welcome back.
I went through your reports. And its good that their is no protein in urine means kidney is not affected.
Cholesterol is high and you just need dietary modifications & few lifestyle change.
About diet decrease fat intake to lowest. If you have to take, use olive oil, stop junk food, stop things that gives you instant energy like potatoes, sweetcorn. Limit intake of sweet & rice.
Drink lot of water & do regular exercise (at least 30min of brisk walk per day). Stop alcohol(if you are taking any). Get blood sugar done regularly & do the tests i told you in previous answer.

Come back if you need any more help.
Thanks & take care
