What Does This Lactose Tolerance Test Report Indicate?
Question: Hi doctor I would like to know what these results mean from my lactose intolerant drink test. I took this test one month ago and its been one month since I ate any hard dairy products. Today, I ended that dairy free diet by consuming fast food fried chicken, which contains dairy. Throughout the month I still felt the same symptoms even having not consume any hard dairy products. The results are in a screenshot form link. https://gyazo.com/bd71be2c0cbb685457b0000ea3d
Brief Answer:
Further data required, still lactose intolerance present...
Detailed Answer:
Hi,
I have gone through your medical history carefully and understood your concerns.
I was reviewing the report of your lactose load testing and unfortunately, noted lactose intolerance as final result.
If the test is done recently, you still need time to apply changes and see improvement.
You report to have the same symptoms, while in the test report it was noted you are not having diarrhea or any of GI problems. Can you please report your symptoms?
Any culture of stool or examination of feces for ova/parasites done so far?
Any upper endoscopy or abdominal ultrasound?
Thank you.

I have stomach problems (growling, noises) and lots of flatulence in the morning. I have these vibrations, shakiness in my legs. My family doctor suggest it is stress and that I should do therapy to clear this. I have done endoscopy, colonoscopy, ultrasound, blood tests, and all these came up negative. I have taken several medications related to stress and antacids and all came up short. I said before how I've been on a dairy free diet for a month now and just yesterday I consumed fried chicken which contained dairy (I took a lactase enzyme pill before my meal).
I am preparing for a stool test for parasites as well
During the lactose test, I have reported symptoms of stomach grumbling and diarrhea that lasted for the rest of the day

Brief Answer:
Seems you need to follow the restriction diet for much longer than 1 month

Hello back,
Sorry for the late reply.
Thank you for providing the requested information.
Your lactose test came to be positive which means that you are lactose intolerant. In other words, all the gastrointestinal symptoms you have are due to lactose intolerance.
Lactose intolerant people can not digest lactose because they lack the enzyme. As this situation will last a bit more than predicted, it is important to follow the restrictions like avoiding dairy or milk.
It's ok to take the enzyme pills as you do but you will feel better if you avoid dairy food completely.
At your age (18), other factors can trigger/aggravate the GI symptoms you have including stress and hormonal. So, as I always advise my patients with the same problems as you, try to give yourself enough time to calm down and take things step by step calmly. Rest enough and sleep properly. Take regular meals and be more physically active.
Continue taking probiotics to stabilize intestinal flora. However, watch if taking probiotics make your GI symptoms worse. Sometimes, it happens so watch their changes.
Try not to eat too fast. Take the pill half an hour before the meal not just exactly before eating because it needs time to start working. Continue with diet restrictions for another month or more to see changes.
You say to get stool examination for parasites. Let me know about the results of the stool test then. That's important too.
Hope this was helpful.

Thing is I am pretty skinny as a result of under eating and overexercising(cardio) in the past and I ate lots of fatty foods and milk to bulk up. This in turn caused all the GI problems I have and have excess flatulence, stomach noises upon waking up and when in quiet situations. If I become physically active, I might fear that I end up being very underweight and my arms are already too skinny. Im trying to adjust my diet a little by adding more and having 5 meals a day. I also am doing only pushups, situps, and weightlifting and no cardio at the moment to see if that does anything better.

Hello dear,
Sorry for the late reply due to my XXXXXXX clinical overload.
Thank you for following up.
Being diagnosed with lactose intolerance and if you avoid dairy food this does not mean that you can not take high calories by other foods.
You can add to your diet more protein with eggs, fish, and meat. Add excessive olive oil or vegetable oil to your salads, nuts, and dried foods, take a lot of carbs, fruits and vegetables that contain good amounts of sugar or fats. Try with avocados, potatoes, legumes, peanut butter, dark chocolate etc.
Also, there are some alternatives for dairy-free milk like coconut milk, rice milk, almond milk and soy milk.
Try to measure your body weight often to see how much you have progressed.
Do not do any excessive physical activity. You can take a walk or ride a bicycle, do things at home and these will be fine to keep you enough active.
Having 5 meals a day is a very good choice from your side. Try to eat more than your body can burn. Do not forget proteins because you need them to form the muscular masses and moderately active. Try not burn everything you eat.
Hope I helped you.
Good luck.

I have one more question. Ive done tests for blood works, colonoscopy, endoscopy, and ultrasound and they all came back negative. Ive taken antacids and antidepressants; none of which are helpful. Me and my family doctor and gas doctors have not discussed much about my stomach acids and thyroid. I sometimes feel these mild burns in my throat along with "frog" noises and movements along them where I can feel it move towards my stomach. Is it possible that i have any thyroid disorders and I dont know how much my previous tests reveal about them.

Hi back,
Now, when a patient with the same GI problems as you comes at my clinic, the very first thing is to exclude GI issues including those related to organic changes (when it comes to anatomy of mucosa) and due to other non organic issues like metabolic disturbances (e.g. those of thyroid).
So, we must make sure your GI tract is ok. From the tests you did, it was resulted lactose intolerance. Therefore, we should focus in improving this lactose intolerance that was already diagnosed.
But this will require time. And you should be patient.
If lactose intolerance is already improved, tests will confirm improvement, and if you will still have the symptoms, then, other tests to confirm metabolic issues are recommended.
All the best!
