Varicocele is a scrotal abnormality . The veins that drain the testicle get elongated, dilated and torturous.

Relationship between varicocele and infertility - The temperature of the scrotum is normally several degrees cooler than body temperature, which is important for normal sperm production and testis function. The dilated veins in a varicocele alter the position of the testis, thus causing exposure of testis to normal body temperature. This reduces the ability of testis to function normally thus causing progressive time-dependent decline in fertility.

I suggest you to consult a surgeon for Treatment of varicocele
The methods are:
1. Incisional repair
2. Laproscopic repair
3. Repair through Interventional Radiology(Varicocele embolization or catheter-directed embolization)
Your surgeon shall suggest you the best suitable treatment of choice.

In semen analysis Report:
Volume is normal.
Sperm count : anything above 20 million sperms/ml is normal.
Since in sperm motility 60% is sluggish (it shows that the varicocele has affected fertility).

At present it is important is to correct the varicocele. After the treatment your sperm motility shall progressively increase.

I understand your concern.
Small varicoceles which donot cause discomfort or infertility are best left alone and only followed up for any increase in size, discomfort or infertility.

Advise to wear tight elastic fibre undergarments is given to patients when thenvaricocele is so small that it neither causes pain nor infertility. But in due course of time the enlarged veins keep getting more larger causing discomfort and affect fertility further.

Some people are known to have varicocele in the same mild condition for years and for some it worsens with time.

However, in your case if your urologist has recommended to wear tight fitting garments then kindly consider his advise and the drug treatment for increasing sperm count may be continued. After the medication when you again get your semen analysis report , based on that your doctor shall take a call on whether surgical repair is required or not.

However, till then some lifestyle modifications shall improve fertility.
1. Kindy lose weight if you are overweight.
2. Donot do excessive exertional excercises or activities.
3. Have a balanced diet rich in zinc, selenium, antioxidants and vitamins.
