What Does This Semen Analysis Report Indicate?
Question: Dear Doctor,
I have been married for two years. My wife got pregnant once (last year), but she had an abortion. Becuase of my work nature, I only meet my wife 2 weeks in a month. However, I did a semen analysis and the results are as follows:
- Volume: 1.5 ml
- Colour: Geryish white
- Viscosity: Normal
- Reaction: Alkaline
- Liquification time: 1 hrs
- Sperm count: 73 million /ml
- Active motile: 60%
- Sluggish motile: 20%
- Dead sperms: 15%
- Abnormal sperms: 5%
- Pus cells: 2-4 /hpf
I did an FSH, LH and Prolactin tests as the doctor suggested. The results are:
- FSH: 0.427
- LH: 5.23
- Prolactin: 239.5 uIU/ml
The doctor said that the results are ok. I asked him about the low FSH and he said that he cares mostly about some figures such as (sperm count, active motile..etc).. I also suspect that the low FSH result is nothing but a typoerror (it should be 4.27 instead of 0.427 as all other results are 2 digits after the decimal) Anyway, I am intending to re-test the FSH once again. Thus, the doctor did not prescribe any medcine.
Another opinion came from my wife's doctor (Gynecologist). She saw my results and she said that my results are not ok. She focused on the "Liquification Time" and she said it has not supposed to be more than 30 mins. She commented on the volume which has to be 2 ml at least. She also commented on the Low FSH and she asked to do the semen analysis once again and to do a testosterone test.
She said that my prolactin is very high, and it suppose to be around 3 ng/ml.
So my questions are:
1. How significant is the Liquification Time ? what should I do to bring it down to 30 mins? is it treatable?
2. How can the low FSH affect my fertility? I mean what are the effects on sperms?
3. Is my prolactin normal (as mentioned in the report)? or it is too high?
Regards

Brief Answer:
Your semen analysis is normal. Blood tests are unnecessary
Detailed Answer:
Hi,
I understand your concerns aboutsemen analysis.
Following is my reply :
1. Liquefaction time upto one hour is fine. There is nothing to worry about it. Increased liquefaction is seen in some men with infection of semen. Hence you can get semen culture and sensitivity test done to check if there is seminal infection and the organism responsible for it.
2. Your semen analysis is normal. Hence all these blood tests are unnecessary. Your FSH report seems to be wrong as you thought.
3. Your prolactin level seems to be fine. There is no need to treat this.
Let me know if you need anymore clarification

Thanks Doctor,
Other questions rgarding the Liquification Time:
1. Will sperms die as the semen get longer Liquification period ?
2. Does the result means I have a constant Liquification Time ?? i.e. if I did another semen analysis, will I get the same result ??
3. After having intercourse, is it preferable that my wife lay down for one hour ?? (since it takes one hour for the semen to be liquified)
Thanks

Brief Answer:
Liquefaction time might change
Detailed Answer:
Hi,
1. Sperms wont die even if you have longer liquefaction time.
2. Yoir liquefaction time wont be constant. If your body defence mechanism has cured the infection, liquefaction time will reduce.
3. If you repeat semen analysis again, your liquefaction time might be altered. Try it.
4. It is advisable to lie down for an hour after intercourse.
I hope your doubts are cleared.
