My husband is facing problem of renal.in blood report creatine syrum is 2.93.and sonography report shows cirtical ecogenecity grade 1 for both the kidney .what does that mean is this serious.do we need to do biopsy...

Serum creatinine is a simple test, and it is the most commonly used indicator of renal function. Normal Serum creatinine level is 0.5–0.9 mg/dL. A serum creatinine of >/=1.4 mg/dl can indicate significant renal disease.
A better estimation of kidney function is however done by calculating the estimated glomerular filtration rate
Normal GFR is around 120 mL/min per 1.73 m2 of body surface area.
There are three grades of renal parenchymal disease.
Grade 1: the Kidney parenchyma is isoechoic to the liver, but there is still cortiomedullary differentiation.
First he has to undergo complete blood count, routine urine analysis and 24 hours urinary protein estimation and other biochemical parameters, some serological tests e.g. ANA( anti-nuclear antibody test) etc, as deemed necessary before embarking on kidney biopsy.

Urine Protein/Creatinine Ratio is commonly used test for kidney disease to determine if excess levels of protein are.present in the urine . Normal range cut off is 0.2 (i.e. 200mg /g )
Urine Creatinine ratio of 3.72 is high. It indicates high proteinuria .
Kidney biopsy:
While renal biopsy can usually be performed in early CKD/chronic kidney disease (stages 1–3), it is not always indicated.
In the absence of a clinical diagnosis, renal biopsy may be the only recourse to establish an etiology in early-stage CKD.
However, once the CKD is advanced and the kidneys are small and scarred, there is little utility and significant risk in attempting renal biopsy,to arrive at a specific diagnosis.
Your Nephrologist would be the best judge. In case,you have doubt,you can always take a second opinion.
