Brief Answer:
Overt Hypothyroidism.

Detailed Answer:
Welcome To HCM. I am Dr Anshul Varshney. I understand your concern.
You are suffering from Overt Autoimmune Hypothyroidism and your TSH levels are extreme high.
You would require a strict control of Thyroid before you plan to conceive.
With this range of Thyroid there is extremely high risk of pregnancy loss.
Moreover your high BP might be because of that only.
I would treat with 100 micrograms Thyroxine for a week and adding up 25 micrograms every week to reach a dose of 150 micrograms per day.
You would require to carry on with 150 micrograms per day dose for next 6 weeks followed by a repeat TSH.
I would also recommend you to take treatment from an Endocrinologist or a Physician and not Gynecologist for this matter.
I hope it helps.
If you have any further query, please ask me.
Stay Healthy.

Brief Answer:
Autoimmune Hypothyroidism

Detailed Answer:
I have mentioned the term "Autoimmune" Hypothyroidism. Your antibodies for thyroid are positive, this means that you have Autoimmune disease. You are most likely to require thyroxine supplement for your whole life.
People who require short course only are those, who don't have Autoimmune hypothyroidism. They might have developed transient hypothyroidism due to some illness or viral infection. Once there thyroid recovers from this, they won't require any further treatment.
As far as Vitamin B12 and Vitamin D levels are concerned, they are low in your case. You can go ahead with the supplementation.
If you have any further query, please ask me, i would be glad to answer.

Brief Answer:
Take supplements

Detailed Answer:
Hi, FT3 and FT4 encompass thyroid hormone regulatory system. The rise in TSH will lead to fall in FT3 and FT4 and vice versa.
You can start with the supplementation's adviced by your doctor. Apart from that, you can take once a week Vitamin D3 60 K supplement.
The foods rich in Vitamin B12 are mostly the animal based non vegetarian products. You can get Vitamin B12 from some nutritional yeasts, as well as from fortified cereals and soy milk.
As far as Vitamin D is concerned, sun light exposure is very helpful. The vegetarian products with high Vitamin D are Mushrooms, fish oils (omega 3 fatty acid).
If you have any further query, please ask me.
