I've gone through your query, reviewed the attached thyroid profile reports, and as per this report, the elevated TSH in presence of normal range T3 & T4, along with your symptoms, is suggestive of hypothyroidism mainly (although there are several other causes behind such symptoms)..
I'd advice you to go for further evaluation i.e. Anti-TPO antibodies to check for any thyroid/immune disorders along with a few basic investigation i.e. CBC, Peripheral blood smear, electrolytes, ECG, Vitamin B-12 and Vit D3 levels, Liver and renal function tests for a complete check-up (all these come under preventive health care check-up plan) to look in to it further..
Management will be based on the findings and if hypothyroidism was found as the sole responsible factor, then a low dose Thyroxin supplementation (Levothyroxin 25 mcg Once Daily in empty stomach in the morning before breakfast) should be started, and next follow-up Thyroid function should be checked after 3 months from now.. All these should be done under proper supervision of either an Internal Medicine specialist or an Endocrinologist..
