What Does This Transvaginal Ultrasound Report Indicate?
Question: Hi,
This is from TRANSVAGINAL ULTRASOUND done today.
Last menstruation is 18 days ago.
ENDOMETRIUM measures 0.73 cm, trilaminar,defined endo-myometrial junction.
RIGHT OVARY: measures 3.5 x 2.5 x 2.5 cm, normal in size with follicles as follows:
F1 = 16.3 x 14.1 mm
F2 = 14.4 x 10.4 mm
F3 = 10.1 x 9.5 mm
F4 = 9.5 x 7.0 mm
and 8 other follicles measuring 3.4 to 6.2 mm
LEFT OVARY: measures 3.2 x 3.2 x 1.9 cm, normal in size with follicles as follows:
F1 = 14.2 x 12.6 mm
F2 = 13.0 x 10.4 mm
and 6 other follicles measuring 6.4 to 6.7 mm
CUL-DE-SAC: no free fluid
Ovulation drug have been given, and ovulation test show positive with 24-36 hours until release.
Can this be a successful IUI if done tomorrow?

Hi,
I understand your concerns about IUI.
Following is my reply:
1. Average size of follicles should be around 18 mm in size before ovulation.
2. None of your follicle size is 18mm.
3. Ovulation test is positive as there are many no. Of follicles which are small.
4. Wait for another 2 days and repeat the scan. If leading follicle is more than 18 mm average size , go for IUI.
5. As of now I dont recommend IUI.

This is what happende:
I made incorrect in my first description - there was not given a Ovulation drug, but an estrogen drug (Menopur 75 I.U.) the day before the IUI. The same evening (8pm) we had a ovulation home test that showed ovulation would be within 24-36 hours (accourding to the test package).
The doctor made IUI the next day (11 am), where there was given an estrogen (and we guess Ovulation drug) named Choragon 5000 I.U.
The post-wash sperm was 30 million with 90% Motility and 85% Progressive Motility - the sperm was given 8:45 am, and kept in good condition by the hospital, until the IUI at 11:00 am.
1. Given that the ovulation drug first was given the day after, would there be enough time to grow the Follicle to better size? (I guess 1-2 day is what we got from the TRANSVAGINAL ULTRASOUND).
2. Is it possible for any of the other Follicle to grow and release egg or will the first/biggest Follicle stop the growth and realse of the other Follicles, when/if it is relased?
3. Can there be an ovulation without release of egg? If so how can that be tested (my wife already have had 2 other kids, so she can produce eggs in the past)
