
What Does This Transvaginal Ultrasound Showing Ovarian Cysts Indicate?

My left ovary states "visualized. size 4.0x4.0x2.6cm mean 3.5 vol 21.8cm3 the cyst was 37x25x33mm with low level internal echos. There was a small amour of free fluid seen in right adnexa in the cup de sac. 37mm cyst.
Obvioulsy I'm nervous. I've had fluid filled cysts before. I was cycle day 6 so that was probably just the normal type. The lefty though, makes me squirm. What does that all mean?
I should have stated that for the past 2 years I have had irregular cycles and cramping for 3/4 of my cycle. No heavy bleeding but cycles varied from 14 days to 48, occasionally I'd get lucky with a few subsequent 26-28 day cycles in there. For the past 5 weeks I've been very dizzy, nauseaus/vomiting, and constantly crampy with full heavy uterus, feels like it is falling out of me. I have cramps down my legs iand in to my back occasionally. No fever. I just want it tostop. We are trying to conceive and it legitimately felt like I was pregnant, but all tests have been negative and I"m on day 49 of my cycle right now.

I understand your anxiety related to the issue.
This seems like a simple follicular cyst and is found in almost 7-10% of patients in reproductive age.
It will disappear itself and you just need a review scan after 3 months.
But since you are having irregular periods and cramping you will need to take oral contraceptive pills for 3 months as a treatment.
I know that since you are trying for conception, this might not sound pleasant to you but even if you will try actively for conception, this cyst will cause poor results.
So just relax and don't panic or get hasty for conception.
As far as cramping is concerned, take tablet hyoscine as and when required and you will feel better.

I don't understand why the right states that it is "simple" but the left is solid if they are both simple as you replied ...and why they are causing pregnancy symptoms, which I have never had with just a simple cyst, which I have had on and off for years. Can you explain how that works?
I cannot take hormones due to very bad reactions that I have to them (had a series of TIA's years back due to hormonal birth control)-even now I get hormonally induced sever basilar migraines.
I have elevated LH levels too if that says anything.

See there are more chances of these being simple cysts only as they are of recent development and hence most likely a simple cyst with slight hemorrhage on left side but not on right side.
It is not a solid cyst but a cyst with internal echos.
Internal echos in a recent cyst has nothing much for worrying but in case it gets long standing then there will be a chance of endometriosis in it.
In case you have had TIAs in past then it will be better if you take GnRH agonist for 3 months after discussion with your gynecologist.
On stopping it after 3 months
- cyst will regress
- endometriosis will be gone (in case present)
- better chances of conception in subsequent cycles.
- PCOD will be less harmful.

The endometrial lining was 5.6mm thick, so no concern about endometriosis and they didn't see PCOD on the ultrasound. Just one cyst on each side, one being simple, the other, being whatever it is.
I hope once this is cleared up I can get pregnant.

In addition to the thyroid panel you need to get AMH and CA 125 levels done.
This is a costly test but will give a lot of information regarding your fertility potential and will guide us for the further management.
