What Does This Ultrasound Report Indicate?

Question: Ultrasound Report - Gravid uterus shows presence of a two gestational sacs. Good decidual reaction is seen around the sac. Thin peridecidual collection seen. Ctrl 1=2.7 mm Ctrl 2=2.8 mm FHR=107B/M FHR=114 B/M Internal OS is closed. Fetal nodes visualized in both sacs. Cardiac activity is visualized in both embryos. Bladder is full with echo free contents and shows normal wall thickness. No andexal mass seen on either side. 2.5 cm anterior wall subserosal fibroid noted. Twin live intrauterine Gestation gestation 5 week 6 days (DCDA).

Now my LMP is 1/11/2015 and as on today i.e. 22/11/2015 I am on 7 week 3 days pregnancy. I have started bleeding in 5 weeks 3 days. Then ultrasound was done on weeks 5 days and the above said ultrasound report came. Doctor advise tranexa 3 times in a day for 3 days and revici injection 2 times in a day for 3 days. After that bleeding stopped for one week and now started heavy bleeding again in 7 week. Again doctor done ultrasound on 21/10/2015. She said both babies are fine and their cardiac activity is there. Now again the same treatment is going on i.e. Revici, tranexa etc. Again bleeding is coming under control. We are under threatened miscarriage case. Kindly let me know whether this is the case of SCH (Subchorionic Hemorrhage). If it is, kindly brief the position of clots in terms of aforesaid ultrasound report of lower abdomen. I have also the visualized pics which can also be emailed to you, if desired. Also suggest the remedies and tests to resolve this issue. Also tell me the chances of miscarriage.

Welcome to HCM,

All complications of pregnancy are slightly more common in twins. But this being a DCDA pregnancy is in your favour, as they are less complex than other types.

Regarding the ultrasound report of peridecidual collection, is what indicates it as subchorionic haemorrhage. Some amount of bleeding and repeated bleeding episodes are common with SCH and it can lead to miscarraige in 10% of pregnancies. But you need to stay positive as remaining 90% are not affected.

So please relax and wait for this phase to pass by. You could consider progesterone either as vaginal pessaries daily or injectable every weekly to decrease the risk of miscarriage.

You could get repeated ultrasound every two weeks to know about the development of the babies and the resolution of the clot.

SCH also slightly increases the chances or preterm labour.

First of all thank you very much for giving a prompt and satisfactory reply. Only two other queries may kindly also be resolved by your goodself.

Kindly let me know whether this ultrasound Report is showing the size and heartbeats rates i.e. Ctrl 1=2.7 mm Ctrl 2=2.8 mm FHR=107B/M FHR=114 B/M, is normal as on 6 week 1 day or not? What should be the size and heartbeat rates of both sacs as on 6 week 1 day in twin pregnancy case.

kindly brief also the position of clots(SCH) in terms of aforesaid ultrasound report of lower ABDOMEN whether it is attached with the sacs or it is above/low of the sacs or it is in the harmful position in terms of both/any sacs. So that the chances of by passing of this SCH stage may be estimated.

Hi there,
The twins seem to be appropriate for the age and you need not worry about their size and heart rate, all are in normal range.
This ultrasound report should reassure you that both babies are fine up till now.
Regarding the clot, it is rather small and its not measured in the scan pictures that you have sent, but it is very small compared to the sac, so most likely it should not be affecting the pregnancy. It is located near one of the sac towards the right, but is very very small.
I know its quite stressful, but the scan report is very reassuring and I am sure such a small SCH will definitely resolve. You need to stay positive.
