What Does This Ultrasound Report Indicate?
There is a mild dilation of the right pelvicalyceal system and the renal pelvis measures 16mm in AP diameter. . There s also very mild dilation of the left renal pelvis which has an AP diameter of 11mm. No ureteric dilation is seen.
The prostate is enlarged with a transverse diameter of 5cm vol 50cc. There is a prostatic impression on the bladder base. The bladder has an otherwise normal appearance.
There is a benign cyst measuring 15mm in the mid section of the left kidney. The renal paranchyma is otherwise normal. There is no cortical scarring or focal cortical atrophy.
There is a calculus in the lower pole of the left kidney measuring 4mm. No other intrarenal calculi are seen. The bladder has a volume of 308ml pre-micturition. There is a large residual volume of urine following micturition of 142ml.
.......what does all these mean? Do I have to undergo surgery?

Thanks for your query ,based on the facts and report of your Ultrasound Scanning that you have posted it appears that you have Enlarged Prostate with significant Post Void Residual Urine with early back pressure changes in kidneys.
I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you have obstructive voiding symptoms like frequency for urination ,urgency ,nocturia,poor urinary stream etc?
I would highlight the report in detail as under
1) Mild dilatation of pelvicalyceal system of the kidneys suggest back pressure changes in kidney due to improper drainage of urine from kidneys in to bladder due to significant post void residue
2)The prostate is enlarged with its volume being 50 cc ,(Normal volume of prostate is 18-20 cc).
3)The cyst in the left kidney is a solitary benign cyst and it is always present since birth and has no significance and not to be worried for .There is no damage to the kidney and it is absolutely normal
4) The calculus of size of 4 mm in lower calyx of Left kidney does nor require any active intervention .It is most likely to be passed out through urine spontaneously .
Enlarged Prostate with significant post void residual urine needs to be removed by endoscopic surgery of TURP to make the urinary passage free of obstruction and to prevent further damage to the kidneys .
This is a simple and gold standard surgery practiced world wide to remove enlarged prostate and offers permanent cure and improves quality of life at old age .

Although I drank a lot of water to wash out the stone, I still have significant pain in the lower back kidney area and front lower abdomen area. There is a clear swelling in the area of lower right abdomen which does not go down even with heat pack, any menthol applications or voltaren. What can be this swelling? Can it be apendicitis?
How soon I need to remove the prostate?
Get operated at earliest to avoid further damage to kidneys .

Could the prostatic and urinary complications as stated by you cause a swelling directly 6 inches under the right breast ? The diameter of the swelling is about 3 inches. This is the most important question I need an answer.

Thanks for follow up .It is very difficult for any clinician to diagnose the swelling over abdominal wall only on description of swelling .It can be confirmed only on clinical examination in person and Ultrasound scanning of abdomen .I will try to opine provided you upload photograph of the swelling with some details of the swelling like Whether it is in abdominal wall ,pulsatile ,hard or soft on palpation etc

By the way, I would like to know the qualifications of your good self as we were not given that information.
the swelling is very clearly seen bordered with a ridge in the photo I have attached. It is directly 6 inches under the right breast. The diameter of the swelling is about 3 inches. It is on the abdominal wall ,not pulsating ,medium hard on palpation.

The medium hard non pulsatile swelling over abdominal wall could be a soft tissue benign tumor like lipoma ,neurosurgical etc .However it can be confirmed only on palpation and examination in person .
