What Does This Ultrasound Report Indicate?
Question: Hello. It always seems that my left nut was bigger and hung much lower than my right. About fifteen years ago I started to have noticible pain from that nut as it often got caught beneath me while driving(manual transmission ,motorcyclying or bicycling. I was in my mid fourties at the time. I received an ultra sound and it was diagnosed as a benign cyst or spermatocele. I was told not to worry about it unless pain increased or became constant rather than based on circumstance. Years later (approximately ten) I was referred to a urologist because my enlarged my enlarged prostate was making pissing slow and I was having frequent urges, and getting up several times a night... I received a prescription for Flomax and that problem resolved. However while examining my left nut (which I had only mild sensitivity to touch) the doctor was pressing increasing hard and I was in agony wondering if he needed to press this hard . I was considering whether I needed to slug him when he said ,"I can't belief this doesn't hurt." to which I said H yes it did. Of course I was not told to tell him if something hurt and I thought there was a reason he was pressing so hard not just to test pain or feeling in general. Well since then approximately three years back it has hurt mildly all the time versus before when it was just a bit uncomfortable when manipulated before. Since more that a decade had gone by and it seemed a bit larger. That testes is bifurcated and probably twice the size of my other testis . The ultra sound I was told showed no growth over the one 13 years back though it did seem a bit larger recently. Since it is mildly painful a lot of the time say two on a ten scale I decided maybe I might opt for surgery. Now I am beginning to think this may not be a good idea due to how long this has taken to develop to where it first became noticeable a long time past and how it didn't bother me much until it was man handed and still is only mildly noticeable most of the time. Also as large( making my nut seem twice as large), I am wondering if there is a likelihood of more adhesion or complication . Since there is a good possibility that much of this will not be covered by my insurance I am wondering if there is a site or way to find what this procedure will cost. Thanks

Thanks for query .based on the history of you having a spermatocele since many years which gives you pain occasionally .
Spermatocele is a benign cyst and you have noticed it to be there since decades I would suggest you to refrain from getting it removed as theer is possibility of recurrence .
However you can decide to go only for cosmetic reasons which I think unwarranted at this age .
ou can take pain killers like Diclofenac twice daily as and when necessary.

1.Will the mild pain stop with surgery?
2. Is it likely to be harder to remove as it has been there awhile?
3. Will the scar bother me as much as the cyst does now? mildly aware of it at all times at present.
4. Since I have had this for awhile and it was not bothering me until it was squeezed so hard.. and it did not grow much over the period...what makes you feel it may grow back?
5. Aside from pain killers do you have any recommendation for exercise or regiment that might help drain some of the fluid involved in making this cyst.

1) It is not 100% guarantee that mild pain will go away after surgery.Infact mild pin does not warrant surgery for removal of cyst which can be easily managed by pain killers .
2) It is not difficult to separate it from the epididymis and remove it even though it is long standing since many years
3) It is a very small scar and will not bother you .
4) Recurrence of Cyst is known and common after surgery as cyst may arise from other tubule carrying sperms from testicle to epididymis (There are multiple tubules
carrying sperm from testicles to Epididymis).
5)There are no preventive measures , diet or exercise that can help to drain out the cyst .
