What Does This Ultrasound Report Indicate?
"Kidneys are normal for size and echostructure.At middle third of right kidney there is a nodular solid formation likely starting from cortical and moderately hyperechogenic compared with the same cortical,having maximum diameter of 38 mm.It displays roundish morphology,regular and sharp edges and,at Doppler fluximeter evaluation,vascular signals at mainly peripheral distribution".(5/17/2017).
I effected, the above ecography as for 5 months I'm suffering for renal colic cured by muscle relaxants and/or Nsaids and water;the pain has'nt been all the day.During the last month I feel pain, nearly all the day, at distal right ureter sometimes very strong that goes away by muscle relaxants and /or Nsaids.The ecographist didn't see any stone,but I feel the stone.At medium caliceal groups of left kidney there is an hyperechogenic spot(2 mm)
There are not hydronephrosis in both kidneys.For about 35 years,every 3 or more years I emitted stones of about 4-5 mm or slightly more, but now I think that the stone is more great(if there aren't other diseases).I've been hyperuricemic for about 35 years or more and I only treated myself by diet(!!!);six years ago uricemia has been 13.50 mg/dl and the last blood chemistry(5/31/2017) yelded normal values except for uricemia that was 8.10 mg/dl and, at urine test, 2-3 erythrocytes at microscopic field.
What is your opinion about the aforementioned nodule(renal carcinoma,angiomyolipoma or other.Can a longstanding hyperuricemia give rise to the before immage,keeping in mind that always I've had pain chiefly at feet and sometimes at "classic" big toe etc. and for 2-3 years pain has been nearly continue and during past 3 months every day,for which I've to use Nsaids.

I do understand your concern
With such presentations in my clinic, I may clarify your answers point wise:
1)The ultrasound report indicate that you have a right kidney mass so you need ultrasound guided biopsy to confirm it whether it is a cyst or tumor and this will clear your doubts better.
2)As per your ultrasound report it does not indicate you have any stone but you feel 2 mm stone so it is very small if present it can be break by lithotripsy.
3)Regarding uric acid now you have near to normal value but you had history of high uric acid level as mentioned by you and it can form uric acid stone also which can be dissolved by medications.
4)Regarding your foot and big toe, I may say that it is due to Gout only. And your long standing high uric acid level cannot be responsible for nodule on kidney. So you need treatment for gout to control uric acid under control and no further NSAID will be required again in future.
