What Does This Ultrasound Report Of Pelvis Indicate?
Multiple images from a transabdominal pelvic ultrasound demonstrate the
uterus to measure 9.5 x 3.9 x 5.8 cm. The uterus is heterogeneous with
several fibroids. The largest one is on the left lateral side of the
fundus measuring 4.9 x 4.4 x 4.5 cm. Note is made of trace fluid within
the endometrial cavity. The endometrium measures 5.5 mm.
The right ovary measured 4.5 x 2.2 x 2.9 cm. The left ovary measured 2.4
x 1.2 x 1.3 cm. There is follicles within both ovaries. There is no
evidence of free fluid in the cul-de-sac.
Possibility of unilateral polycystic ovary and fibroids to be investigated
Detailed Answer:
Hi XXXX,
Thanks for writing in to us.
I have read through your query in detail.
Please find my observations below.
1. The ultrasound report shows that your uterus is enlarged in size and has many fibroids which are usually benign tumors which might be related to hormonal imbalances.
2. The scan report also shows that the right ovary is enlarged with a volume of 14.99 cc and shows follicles. The left ovary is normal in size. In this regard, the possibility of a unilateral polycystic ovary pattern involving the right ovary might be considered which is an outcome of hormonal imbalance.
3. Unilateral polycystic ovary might be the cause for your weight gain and symptoms including hot flushes, palpitations, panic, sleep disturbances and nightmares.
4. I wish your doctor investigates you for polycystic ovary disease after matching the ultrasound findings and then addresses your concern.
Other symptoms of polycystic disease includes irregular periods, infertility, acne and excessive hair growth on the body. Please note that my opinion is based on what is written in your query and clinical methods require to be applied before making a confirmed diagnosis.
Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.
Hope your query is answered.
Do write back if you have any doubts.
Regards,
Dr.Vivek

Detailed Answer:
Hi XXXX,
Thanks for writing back with an update.
1. The uterine fibroids can be treated surgically or by minimally invasive embolization technique. Since the exact number of fibroids is not mentioned, I consider that there might be several of them with the largest measuring 4.9 cms in diameter. Surgery is recommended when the fibroids are too many and causing severe pain, bleeding and discomfort.
2. Polycystic disease of ovaries is treated by hormones and nutritional management. This problem was earlier considered to be prevalent in the younger women but with progress in medical science, it has been observed that polycystic ovary disease manifestation might continue through menopause. This also includes treating other associated conditions like heart disease, diabetes and mental stress, should the patients have such problems.
3. I am aware of your menopausal age group. Should it be confirmed clinically to be having polycystic ovary disease then nutritional management will help to manage your condition.
Hope your query is answered.
Do write back if you have any doubts.
