Posted on Thu, 23 Jun 2016
Question: OK, Just searching the web to figure out what could cause 3+Leukocytes/3+RBC's in the urine=no growth on culture X2 visits & 2 different atb's. I do have some cramping, frequency, & did one day have visible blood in the urine. Kinda freaks me out since I am overall a pretty healthy person= 63y/o. Any ideas?
Not sure what would be helpful with this question.
I forgot to mention that I have Hep C
US/Fibroscan neg.w/slight fibrosis & mild scoliosis.
Am hoping to get the new Hep C medication but not easy to get the insurance cover it. I have an appointment w/Gastroenterologist @ UCDavis next week as fu to the US/Fibroscan.
Hate to go to see a urologist & have them do any probing around so open to some alternative ideas. Am not anxious to take drugs or have proceedures done.
Am drinking fresh mint in filtered water.
Juicing fresh lemons for lemonade.
Melaleuca tea.
Hope you have some good ideas :)

Brief Answer:
sterile pyuria likely

Detailed Answer:
Thank you for asking

I read your question and i understand your concern. 3+ leukocytes esterases with negative culture results make it likely a sterile pyuria and should be treated and managed conservatively with supportive care you are already on and combination antibiotics. I would recommend fungal cultures as well though. With hepatitis c and immune modulating medications, fungal urinary tract infections can be a possibility.

As far as with hepatitis c and slight fibrosis, you are a good candidate for new advanced antivirals like sofosbuvir etc. Let your doctor find your hep c genotype and start you on these antivirals, these have very good response rates upto 98 %.

Nutshell, urinary symptoms should be treated for sterile pyuria and hep c treatment should be started with immediate effect ASAP. I hope it helps. Take good care of yourself and dont forget to close the discussion please.

Khan

Thank you Dr.
Hep C genotype confirmed & hoping to get the new medication for complete cure in 8 wks.
Re: Sterile Pyuria, good call! More than likely this is the cause.
On-line search gives no solution to this problem.
Occurrence can be due to viral Hep C/ Herpes then really is there anything else I can do to help myself? Viruses are not easy to combat.
Would anti-viral drugs be of any help? i.e.; Valacyclovir 1 gm. tablets
I will continue the routine HRT, Melaleuca Tea/Douche rinse, Increased fluids, & little to no sugar. Would you also recommend Urologist? or Just wait & see if this resolves itself.
Not willing to take atb's since the Cipro & Cephelasporins were not effective.
Look forward to hearing from you again, XXXXXXX

Brief Answer:
Supportive care

Detailed Answer:
Thank you for getting back to me

Its good that hep c genotype confirmed, with the advanced medications, remission rate is 99 %. Sterile pyuria is likely possibility, just good toilet habits, frequent fluids intake. cranberry juices and garlic intake, avoid douching, wear clean loose undergarments . Yes a visit to a urologist will help for further working. As i said fungal cultures should be done as well as other possible cultures. Waiting and seeign and supportive care will help for sure. No need for antibiotics for a while now and continue your rest of the treatment .

Thank you Dr.
I take friendly bacteria caps, eat yogurt daily, Melaleuca Tea…since I have to wait for referrals to process, what about taking an oral anti-fungal drug like Diflucan?
OTC Monistat? to cover the possible fungal cause of these symptoms.
Since the urine culture was negative twice then it must be fungal, right?
What else could it be?
I understand that with Hep C you are predisposed to skin problems & I do have confirmed pathology for Lichens Planus/Sclerosis. Maybe this could be an inflamation caused by the Lichens? Just guessing!
Interested in what you think.
Thank you.

Brief Answer:
Need work up and management accordingly.

Detailed Answer:
Thank you for getting back to me

There is no harm in taking a dose of antifungal like diflucan . With antivirals, immune system is vulnerable to even opportunistic organisms. Rest there are many possibilities for it and need a complete clinical correlation. Repeated cultures of urine and blood, with focus on bacterial, fungal and parasites all should be considered. With lichen planus a diagnosis, it could be a cause for itch and inflammation. So needs a thorough evaluation before connecting the dots. let your doctor decide what is best for you.
