Question: Hello, I got a Widal test in Bolivia where we have been living for 2 1/2 months. I have diarrhea off and on and occasional fever around 37.4 C. I only drink from the 20 liter bottles delivered to our home, and soda, wine or beer. My results on the lab printout are cryptic. The doctor explained in Spanish to me that I may have an ongoing infection and put me on ciprofloxacina for 7 days. The doctor said to boil all our water, even from the 20 liter bottles. Leukocytes were 11,700 where the upper limit of normal is 8,000. All else normal.
O NEGATIVO
H POSITIVO 1/40
PA NEGATIVO
PB POSITIVO 1/40
Sed rate value normal range
1 hour 20 mm 1-10 mm
2 hours 40 mm 0-20 mm
Index of Katz 20 mm 0-10 mm

All other results from CBC normal
Thanks, XXXXXXX

Brief Answer:
Findings suggest positive widal test.

Detailed Answer:
Hello XXXXXXX

Your reports suggests positive widal test.It indicates typhoid fever. Its titration for positive results varies with laboratory.
A single Widal test is of little clinical relevance due to the number of cross reacting infections. However, four fold increase in serial titration is confirmatory.

You have been prescribed right medicines and you should take it regularly. You are rightly advised for preventive measures like to take boiled water.

Hope i have answered your query.

1. Do you think I should get a followup Widal at some point, or just see if my symptoms cease after finishing 7 days on ciprofloxacina 500 mg twice a day?
2. I am curious as to why a radiologist was the one to answer my question instead of a clinician/general practice physician.
Thank you.
