What Happens In Stage 4 Prostate Cancer ?
Question: My husband has prostate cancer T4N2 and is on hormone therapy , has had a radioistoble scan.
This is in Edinburgh Scotland.
What happens now?

my name is Dr S Khan and i read your concern about the terminal stage also known as stage 4 prostate cancer of your husband. Miss XXXX i am sorry to say but stage 4 is the high grade terminal prostate in which Tumor is fixed and has invaded adjacent structures other than seminal vesicles (eg, bladder neck, external sphincter, rectum, levator muscles, pelvic wall. And N2 means not only regional but distant lymph nodes are involved, the place where surgeries and odds for clearing the malignancy with surgeries are impossible. SO now i am afraid conservative therapies and symptomatic care is the only option you have. Miss XXXX i am really sorry for your troubles and i can feel the pain but its the nature and we have to face realities even if they are harsh.
I want you to stay calm and stay in touch with your oncologist and let him decide what is best for your husband. Alleviating discomfort and treating symptoms is priority. Discuss androgen suppression, radiation therapy and palliative care about your doctors.

I am a slightly confused as he is being treated at Urogoloy and when diagnosed the consultant said that the homorne therapy could go on for a while before radiotherapy/chemo would be involved, but I got the impression that there were more concerns about the spread elsewhere and into the bones.
There was no mention of oncology at the time of his stay in hospital, but have been aware that is may be more serious than when first told
We are still waiting for the results of the radioisotobe scan,
I thank you for your frankness and when we next see any GP or Consultant I will be asking more probing questions.

Thank you for getting back. Yes the doctors are right. it is the protocol they are following. Don't be confused. Hormone therapy is being tried but if the disease still progressed the medical oncologists for chemotherapy would be involved for intervention. And radiation oncologists for radiation treatment with different modalities. They know very well what they are doing. You should just be in touch and keep your self up to date with the patient education programme and keep me posted too when the results of radio isotopes scans come up.
