Both manic spells / anxiety as well as depressive episodes of bipolar disorder as you may know are from neurochemical (neurotransmitter) disturbance. Two group of chemicals that cause opposite symptoms need to be balanced which is what is altered in bipolar disorder. Besides these chemical imbalance there are no other structural damage in the nervous system.
If the condition isn't treated, these disturbance become chronic where mood fluctuations are too frequent. There are higher chances cognitive impairment and psychosis.
