Brief Answer:
Fatty liver disease leading to liver failure and need for transplant.

Detailed Answer:
Welcome to HCM: Please note that and enlarged liver can be caused by;
1. Long time alcohol use
2. Obesity (causing fatty liver disease)
3. Hereditary
4. Alpha 1 antitrypsin deficiency
5. Hepatitis that is untreated (hepatitis A,B,C,D,E)
6. Parasites live liver fluke
7. HIV can also cause an enlarged liver
8. Carcinogens like tobacco and dyes/industrial

As you can see this is a very long list which is obviously not all inclusive, therefore a directed investigation needs to happen with your regular doctor.
