I read your query and here is my advice.

You are saying that you have consistent uti but I want to know how often you had it and what have you used.
I think you should do an uroculture before taking antibiotics.
You can upload the results to me to see the antibiograme and select a sensitive antibiotic which can be used in persons allergic to penicillin.
I would have treated you empirically with ciprofloxacine or levofloxacine for 7 days.
Also continue to drink lots of fluids and especially cranberry juice for a month. Cranberries contain a sugar which fights bacteria.
If symptoms still persists I can give you nitrofurantoine low dosage for a month.
