Question : what is likely cause of diffusely thickened bladder wall and scattered echogenic foci mean? Some intermittent night time incontinence. Same ultrasound showed enlarged spleen. 7/3/18. Had one in April, 2018, same thing but sedimentary layer, no enlarged spleen. Low WBC. Flushing face. Constant headaches. Insomnia. Breathing problems. Older teen boy. Why hasn't urologist done cystoscopy? Had MONO/EBV over a year ago, high viral load. Dx POTS. low blood pressure, taking fludrocortisone, also. Lost a lot of weight. Slightly high RBC, also. LUPUS? ANA negative. Cancer? Next steps?

Anyway, if this helps since there is a history of infection in the past, and enlargement of the spleen, those ultrasound findings are consistent with cystitis, (cystitis cystica, eosinophilic cystitis, emphysematous cystitis).
Often cystitis may mimic a bladder tumor and ultrasound alone is not sufficient to achieve a correct diagnosis.
Cystoscopy with biopsy is necessary for a correct diagnosis.
Discuss with your Doctor about these issues.

There was no infection from the urinalysis. the hx of infectious ebv mono was over a year ago and he had enlarged spleen but that has resolved.
