Question: Ultrasound of abdomen says that liver is echogenic and attenuating consistent with diffuse hepatocellular disease. There is an ill-defined hypoechoic area adjacent to the middle hepatic vein, measuring 1.5 x1.2x1.5cm. No intra or extrahepatic biliary dilatation is identified. Main portal vein demonstrates Hepatopetal flow.

I read your question and i understand your concern. Your ultrasound reports likely of a fibrosis or liver architecture also called cirrhosis or a fatty infiltration.

That ill defined structure needs further evaluation. I would go for a CT scan to see what is going on.

Bile ducts are fine as there is no dilation noted. So what ever it is it is inside liver.

Also the portal vein circulation is fine and the portal hypertension or reversal of flow which usually occurs in cirrhotic patients is not there.

Nut shell It needs a complete clinical correlation and a thorough assessment with history to sort out what it is. Likely possibilities are of a fibrosed liver or a fatty one.
