Hello,
Based on the fact that you have been diagnosed to have Enlarged Prostate and your Ultrasound Scanning has revealed PVR of 70 cc,
In a normal individual under normal circumstances bladder is completely emptied during act of micturition and no urine remains in bladder ,However whenever prostate is enlarged there is obstruction to the outflow of urine which results in to incomplete emptying of bladder and some amount of remains in bladder what is called as Post Void Residual Urine (PVR).
This PVR in terms of volume determines the grade of obstruction to urine outflow and to decide the modality of treatment .A PVR of 70 cc suggests severe obstruction and needs to be relieved by a surgical treatment of Trans Urethral Resection of Prostate (TURP).
.PSA above 4 is considered to be due to Cancer of the prostate unless proved otherwise by doing a Trans Rectal Ultrasound Guided biopsy of Prostate .However it is also increased in Enlarged Prostate and infection of Prostate .
Raised PSA with enlarged prostate lowers down and comes to near normal value after taking appropriate antibiotics for 2-3 weeks .Hence if the PSA is higher associated with enlarged prostate it is a normal protocol to give antibiotics and combination of Tomsulosin and Dutasteride once daily for 2-3 weeks and repeat blood tests for PSA after completion of course of antibiotics after 2-3 weeks .
