Question: Hi. I'm 28 years old. 65kg. with good blood pressure.
I got a problem using handcreams on what started to be a hard skin on the side of the penis. I thought the handcreams made it worse.
Then after i tried to put olive oil on it, But it too didn't help.
It grow fast now each day. And it have grown to red dots on the skin. Also as red bags under the head of the penis. Lots of white fluid strong-smell acid-like smell comes out from this area i think. Some bruises have shown up around this area too.
It have a white-ish color inside like as it was the source of the acid-like fluid.
I have had much sex lately with a new girl. And it happened i got to get out a wisdom tooth at the same time as this worsened. And i got fever from the pain of the tooth i think just before. I have also been under some stress lately.
Before i started having sex with her i have tested myself for all sexual transmitted deceases, and she say she don't have any also.
The pain is very great now, and i'm not able to open up the head to look under it. the skin have become hardened. And i was thinking about trying out hydro-cortisone.
Thank you

Hello,
Welcome to healthcare magic,
From your description it appears that you have some sort of infection, probably an STD. It is unlikely to be due to hand creams. If you can upload a photograph of the lesion it will help a great deal. You have a feature to upload the images by yourself at the right side of the query page, please utilize that so that I can answer your query better. You can also send the image as attachment to YYYY@YYYY , with the subject as 'Attn: Dr. Johny Chacko'.
Do not worry about privacy as this website takes utmost care about it. It is better to avoid applying hydrocortisone till we diagnose the condition as steroid is not suitable for infections and works on allergies.
Take care.
thank you for your reply.
I forgot to mention that we were using baby oil (natusan). and i think i had a reaction to it. i did some mistake when i just wanted to attempt to "heal it a little".
1. firstly with badumecum (mouthwash that can be used to heal wounds).
reaction: hurted much.
possible sideeffect of that was redish dots..
2. hand cream eldena.
possible sideeffects: possible a growth of bruses and red dots.
3. lastly i tried with hydrocortisone.
seems like it turned all the red dots into open wounds.
Now i think i wont try any more things. Just let it heal in the open air.
It hurts really bad. and if i have it closed, it generate fluid that hurts too... i don't know what this do.
After i washed it very long, it seems the fluid that comes is more blank than white-ish.
only the open wounds seem to be a little white-ish.
I will send a picture, which was taken after step 2.. Thank you

Thanks for the photograph

Hello,
Welcome back,
The clinical photograph which you have uploaded looks like a viral infection called herpes (Genital Herpes). It is a sexually transmitted infection. Usually it starts as painful red area, then grouped fluid filled blebs or blisters develop which ruptures to form shallow painful ulcers with a scalloped margin. Your ulcers look like a typical herpetic ulcer clinically.
Treatment of Genital herpes is T. Acyclovir 400 mg three times a day for 7 days for the first episode and for 5 days for recurrent episodes. But this tablet has to be started early. Now if you start in your case it will not be useful.
Can you tell me exactly how many days back these lesions started ?
You may do a blood test to check HSV 1 & 2 IgG and IgM which will help. Now apply only an antibiotic cream like neosporin or mupirocin 3 times a day over the ulcers. You may take T. acetaminophen 500 mg three times a day for pain relief. The ulcers will heal in a few days time.
Hope this helped,
I will be happy to answer any further clarifications.
Take care.
Thank you for these good suggestions.
They started about 4 days back very small, and just exploded after that.
It sounds very logical your explanation.
I'm going to be tested for those deceases and get one of those creams.
It seems they start to heal a little when i give it air. and the fluids isn't developed as much as before.
I forgot to mention i had exam in the face that i use hydro-cortisone on. Also i have a skin intolerance to chlorine(when bathing). But not any food intolerance according to earlier blood tests.
Brief Answer:
Herpes Genitalis
Detailed Answer:
Hello,
Welcome back,
Acute history of 4 days is again consistent with genital herpes. Initially it will be more painful and oozes more fluid and gradually in one week to 10 days time it heals. This is unlikely to be due to chlorine water or food. Do not have sexual intercourse when you have active herpes.
If required you may contact me to interpret the test results.
Take care,
Wish you good health.
i try to hold it so it all get air. then no more fluid; and it all seems to heal.
If it heals quickly now, is it safe to say it's not genital herpes?
Thank you
Brief Answer:
Genital Herpes
Detailed Answer:
Hello,
Welcome back,
Even if it heals quickly clinically it looks like herpes. Usually herpes lesions heal between 5-14 days spontaneously. It is better to do the test. You can do the test after a week so that antibodies will develop by then and you will get accurate results.
If you can get back to me with the result, then I can interpret it and counsel you about the disease, its recurrence chances and precautions to be taken.
Hope this helped,
Take care.
