Brief Answer:
Not SFI

Detailed Answer:
Hello,
I can imagine your plight. It is bad enough not being able to sleep for a week. On top of that your internet search has landed you on a fatal disease. SFI is an extremely rare disease and only a handful of cases have been reported proven even though it was first described in the 18th centuary. It is extremely rare and you are unlikely to have it as it is also an inherited disease ( autosomal dominant) which means that someone close to you was very likely to have had it. The fact that Temesta which is an anti anxiety medicine ( Lorazepam) had helped you, in spite of its side effect, means that the insomnia is more related to anxiety. In SFI it would not have worked.
So be relaxed and you are more likely to sleep. Your neurologist would probably suggest some simple measures to help you sleep.
I shall be interested in knowing how you progress and if you have any queries you are welcome to ask.

Brief Answer:
Don't worry, relax.

Detailed Answer:
Dear XXXX

You are so keyed up that metatop is not able to calm you down adequately.
So relax, try some simple measures like reading , to keep your mind off the insomnia and sleep would come. I do not prescribe benzodiazepines to my patients as people tend to get addicted and would require higher and higher doses. I sometimes advise a simple sedating antihistamine like Benadryl (diphenhydramine) for my patients.
Most important is not to worry and try whatever measures you normally do to relax.
Best wishes,
Dr. Noble

Brief Answer:
May take with care to avoid side effects.

Detailed Answer:
Dear XXXXXXX

I am sure he meant well. But since you had developed abdominal symptoms and vomiting last time, it would be better to take a proton pump inhibitor like Losec or Nexium and domperidon ( Motilium) about half hour before the temesta, if you want to take it. If you don't mind taking the risk of not sleeping one more night, I would prefer you to relax and take some home measures for inducing sleep. You are aware that you should avoid all stimulants like caffeinated beverages etc.
Wishing you a very restful night and good sleep,
Dr. Noble Zachariah

Brief Answer:
Esophageal disease to be cared for.

Detailed Answer:
Dear XXXXXXX

For esophageal ulcer or inflammation, the correct dose of omeprazole is 20 mg twice a day on an empty stomach ( at least half hour before the next meal). To this you can add the domperidone . Also keep the head end of your bed elevated by 10cms with blocks under the cot legs. Elevating the head with pillows is counter productive as it increases the pressure in the abdomen leading to acid entering the esophagus from the stomach. Your last meal and drink should have been at least 2 hours before the bedtime so that you go to bed with an empty stomach. Avoid all fizzy drinks, spicy food, chocolates and sour acidic food like tomato, onion, lemon etc. Acid regurgitation also can interfere with your sleep.
If you are overweight or obese losing weight also is beneficial.
Hope you get well and happy soon.

Brief Answer:
Glad that the neurologist confirmed

Detailed Answer:
Dear XXXXXXX

I am glad that the neurologist confirmed that you do not have SFI and that your EEG is normal.
I agree with her short term prescription of Xanax and Rivotril (? strength) and I concur with her opinion that with time you would return to your good old self.
If you do not get sleep for any reason, try to utilize the time to do something useful rather than worrying about not getting sleep.

If you have no further queries at this point, you may close this discussion and record your comments.
Best wishes for your good health .I am available if you require my opinion any time in the future.
Regards,
Dr. Noble Zachariah
