Question: Urological/Nerve damage problem??? This trauma was caused by a Kenalog injection to the penis to alleviate pain under a previous scar from a Penile lesion resection surgery performed in 2007..Huge mistake. The Kenalog injection caused penile tissue to swell and most likely ate healthy tissue (Caused penile atrophy) and affected peripheral nerves.. I have been experiencing sever nerve pain for the last 2 years. Please help!!!

Brief Answer: The nerve on lateral side appears to be damaged. Detailed Answer: Hello Thanks for your query,based on the facts that you have posted it appears that the nerve on left side of your penis is damaged due to injury caused by injection Kenalog (Triamcinolone)given on lateral side of the penis. I shall be happy if you can post me few more details to help me in making comments on your problem. 1) What was the lesion on Penis that was operated for in 2007? 2) Was the scar resulting out of first surgery painful and hypertrophied ? 3) Was the person who has given injection on penis is qualified and specialist in this field.? Normally Triamcinolone is given intramuscular or Intra articular and never given intra dermal It seems that the injection given on lateral side of penis has injured the nerve located on lateral side of penis. Since any nerve injury takes long time to regenerate or may not regenerate at all.It depends upon the extent of damage to the nerve and regeneration of the nerve can not be guaranteed. Please get nerve conduction of the penile nerve test done to assess the extent of damage Till then you have to take pain killers like Diclofenac or Bruphen twice daily. Dr. XXXXXXX Thank you for your reply.. Yes I agree with you that the nerve on the lateral side is damaged.. I also believe that there is blood vessel damage in that area.. When I take medications like antidepressants, Cealis, and high blood pressure medication, that open up blood vessels and increase blood flow, the pain increases significantly...Excruciating!!! Questions for you and then I will answer your questions that you have posed to me.. 1) The damaged seems to be in the top left corner of the shaft and the swollen tissue wraps around toward the back of the penis shaft.. It also is swollen lower down the left side of the shaft. These areas actually look engorged.. Do you think I have peripheral nerve damage in just these areas or is there damage to the dorsal nerve? 2) Are peripheral nerves a branch of the dorsal nerve or the pudental nerve? 3) Do you agree that there is most likely blood vessel damage also? I certainly think so.. 4) You are the only Urologist that has stated to get nerve conduction testing... I have seen many and they all tell me that this is not possible in the penis??? Where can I get this nerve conduction testing?? 5) Are you still a practicing Urologist? You seem to have more knowledge than all of the top Urologists I have seen... If so, I would like to come see you where ever you are located in the United States...Please let me know your location so I can set up an appointment to come see you... 6) Can you do this nerve conduction testing? 7) I got this Kenalog injection Oct. 2nd 2012, and I have been in excruciating pain ever since, so I believe the nerve(s) are not going to repair themselves and regenerate... What solution do you propose to solve this problem? Microsurgery to repair damaged nerve(s) and blood vessels?? Is this even possible?? Now I will answer your questions... 1) The lesion that was resected in 2007 was some kind of growth that was not cancerous.. It left me with a scar across the shaft of my penis right below the head of my penis. My penis was reconstructed.. 2) The scar did hurt for a very long time after surgery.. I do not believe it was hyperthrophied, but there was definitely scar tissue development under the scar.. 3) The doctor that gave me the Kenalog injection was NOT a qualified specialist in the field of Urology.. He should have never injected a cortisol steroid into my penis...!!!!! Outrageous!!!! Please get back to me as soon as you can.. Brief Answer: Neurovascular bunle is damaged in first surgey. Detailed Answer: Thanks for follow up. I am practicing in India so it is not feseable to meet me personally. From all the facts that you have posted it a[[ears that the neuro -vascular bundle on left side which contans branch of Pudendal nerve and blood vessels are damaged during first surgery itself causing fibrosis and scarring which is giving all these problems that you have. Truly speaking Injection Kenalog should not have been given which has damaged the neuro vascular bundle further and has aggravated the problem further. The chances of natural regeneration of nerve seem to be blink, however you can consult qualified Neurosurgeon to explore possibility of any hopes with micro neurosurgery to repair it . Dr. XXXXXXX Where in India are you located?? I am willing to travel anywhere in the world to have this problem fixed.... With this said, can you fix this problem or know of a great Neurosurgeon in India that could fix this problem?? If you give me your phone number I can call you for a phone consultation whenever you have the time and when it is convenient for you..??? Or a top Penis Neurosurgeon in the United States that could fix this problem?? I live in the XXXXXXX AZ area.... I am practising in Banagalore and to be honest have not come across such cases in my 40 years practice .I understand your misery and concern but I would advice you to find out some one in U.S who deals with such cases .
