Question: Hi doctor I had my pregnancy test and confirmed it. As we are not financially stable so we decided not want a baby at this. I just got confirmed 5 days back so could you suggest me how to use the mifepristone and misoprostol dosage

May I take the XXXXXXX to advise you. I suggest that if you are planning to have baby later, you might as well continue with this pregnancy only. You both should think about this seriously. In my practice I have seen some couples terminating pregnancy and later suffering from infertility. So I thought it is my duty to advise you on this.

Now coming to your question, before taking any medicines to terminate pregnancy it is extremely important to confirm that the pregnancy is within the uterus. If pregnancy is outside the uterus, with these medicines, it can burst and cause life threatening internal bleeding. It is also important to make sure that you do not have contraindications to taking these medicines. So please consult doctor for assessment. You need to get an ultrasound done to confirm that the pregnancy is inside the uterus. Without this baseline assessment it is unethical for me to advise you regarding how to take these medicines.

I insist that you should consult doctor before using this kit. I have already explained the dangers of self medication above.

I am writing how to use it as you are insisting for it.

First take 200mg of Mifepristone orally.
24 hours later 800micrograms of Misoprostol should be used vaginally.Most women ( 80%) abort within 24 hours of taking the second pill. Some take little longer. After aborting the pregnancy you may have light bleeding for two weeks. Get an ultrasound done after two weeks to make sure that the abortion process is complete.
If you have heavy bleeding (soaks 2pads/hour for more than two hours) or if the bleeding persists even after 2 weeks you must see the doctor.

To avoid such unnecessary stress I advise you to consult your doctor and start regular contraception like hormonal contraception or have an intrauterine devise fitted as they are very effective in preventing pregnancy.

These 800 mg misoprostol tablets used vaginally should be taken at a time or one by one? There are 4 tablets each of 200 mg so after taking mifepristone before what time i have to take the first dosage of misopristol and later how many. Kindly suggest me the usage of misoprostol.

I ate raw papaya three days back. From two days, everyday morning i am suffering from vomitings and drowsiness. Can i expect any side effects if i take mifepristone and misoprostol tablets now or shall i wait for few days to take tablet ?

The vomiting and drowsiness that you are suffering are not due to Papaya, but due to pregnancy.

Mifepristone usually does not cause side effects. It may cause mild cramps. If cramps are severe, you can take Brufen ( Advil) 400mg every 8 hours.

Misoprostol may cause severe cramps, vomiting and diarrhea.
For cramps you can use Advil. If vomiting is severe, use Benadryl 25mg or dramamine 50 mg 6th hourly as needed,
If diarrhea is severe, use Imodium 2mg once.

If you decided to terminate the pregnancy, earlier the better. So use the tablets now.

I had my dosage of mifepristone and suddenly started bleeding. Is everything going right because iam about to take my next dosage of misoprostol. In the mean time heavy bleeding is started so can i take misoprostol orally because iam unable to insert them now. So please kindly inform me what to do know.

Sorry could not reply to you immediately because of time zone difference.

How is the bleeding now? I am sure it has reduced now. About 4% of women abort with Mifepristone alone. Probably you have started the abortion process. You may not need Misoprostol. But without ultrasound and examination it is difficult to say so. As I have written above, if you are bleeding heavily you need to go to the hospital.

If you are bleeding too much you can take Misoprostol orally. Take 400mg twice at an interval of 6 hours. The disadvantage of taking orally is that the side effects can be severe.

Right know i dont have any bleeding it stopped. Blood tissue has come out at timeof bleeding.So do you want me to take the misoprostol orally or insert into vagina? Let me and i will make sure to take an ultrasonic later on. And suggest me what are the precautions to be taken at time of taking the medicine and after taking it too.

Yes if you have no bleeding use Misoprostol vaginally.
I suggest that you get an ultrasound done now. If the abortion process is complete there is no need to take Misoprostol.
