What Is The Dosage Of Banocide For Wheezing In Chest?

Question: Hello Doctor.
We have consulted you before as well and found your advise very helpful at that time. Therefore opted to ask for the advise this time as well.

My son, 2 year 2 months, was having cough / cold / high fever since 22/09/2015

Treatment received so far:
Syp Dolo 250 3.5 ml thrice a day -- 22/09/2015 - 25/09/2015
Syp Ambrodil 200 3ml twice everyday -- 22/09/2015 - 27/09/2015
Syp Atarax 2ml twice a day -- 26/09/2015-27/09/2015
Syp Azithral 200 3ml once everyday -- 24/09/2015 - 28/09/2015

Symptoms / changes:
High fever -- 22/09/2015-25/09/2015

Low grade temp -- started on 26/09/2015, the underarm temperature has been staying between 96.1 - 97.4 (without adding +1 to determine internal temperature),
he feels colder than normal, sweats a lot if he plays, in general sweating has been increased

Cough -- started on 23/09/2015, started subsiding on 25/09/2015, at present it has reduced a lot

Cold -- started on 25/09/2015, mostly runny nose with little bit of mucus, subsided but not cleared yet


Doctor who prescribed the above treatment said that there is no need to give any further medicine except the ORS as he may be loosing water through sweating and hence the temperature drop.

We never do this but consulted other doctor to see if he needs further treatment.
She prescribed Tixylix and Banocide citing the minor wheezing in the chest. She did not mention about the specific variant 120mg/50mg of the Banocide. I was not aware of it either. And purchased one from the pharmacy which was of 120mg/5ml strength.

We gave him 2.5ml Banocide twice today - 11:30 AM and 4 PM
(it was prescribed as 2.5 ml thrice by the doctor)
I realized that it is an overdose after reading more on Banocide online.

Present condition:
Today he slept for little longer but has been playing since 5 PM till now.
He is active- playing, eating well, motion is regular, urination 3-4 times today regular. Little but coughing and runny nose with very less mucus.
I have stopped all medicines after realizing the Banocide overdose and started giving him more water (post 8:30 PM). He has urinated once at 6:30 PM.

Questions:
1) Should we be concerned about Banocide overdose? What remedy we can initiate right away? Should we see doctor right away?
He is interacting with us properly, has played and taken food properly as well.

2) Should we be concerned about low temperature? How long it will take for it to come back to normal? What care we have to take to handle the low temperature? Should we go by the temperature alone or look for clinical symptoms as well for warning sign?

4) I will attach his CBP / CRP reports (done on 28/09/2015) sometime today. Just mentioning the summary here:
CRP was negative.
Lymphocytes 70% with the reference range of 20-40%.
RBC 3.9 Mill / Cumm (Range 4.5 - 6.5)
WBC 6100 / Cumm (Range 4000-11000)
Platelet count 2.4Lakhs/Cumm (Range 1.5-4.5 Lakhs/Cumm)

Does this report indicate anything? Doctor was highlighting the Lymphocytes part. What does that mean? Is he still having the infection or it will go away without any medicine?

5) Will ask further questions, if any, once I hear from you.

Hi...by what you quote I feel that your son is recovering from a viral illness.

I am coming to your queries directly -

1. You need not worry about that Banocide dose. You have already done the best possible thing - giving him more water. You need not immediately rush to a nearest emergency room as he's normal and interacting well and acting normally.

2. You need not worry about the low temperature. Ups and downs in temperature are common in recovering viral illness. You need not do anything as of now to increase the temperature as he's normal and active.

3. Warning signs of low temperature - bluish discoloration of lips and finger tips/ loss of consciousness/ low urine output/ rash over the body and limbs/ seizures/ inability to recognize parents or family members. If these are not there and if he's otherwise active, don't worry.

4. The reports are suggestive of a viral illness. The lymphocytes increase in number is suggestive of the same. CRP being negative is Good.

5. This will go away without any further medicines. Do not worry.

Hello Doctor,
Thank you very much for the quick response. I was very much worried as it was an overdose scenario. I am stopping all the medicines now (infact, I have already have since the last dose at 4 PM). Also, my son has been active/alert at 11 PM and went to bed after that.

Few followup questions / more info on the illness:
1) 1 or 2 days after my son got these symptoms, myself and my wife also got contracted with the similar symptoms - throat pain/fever/coughing.
It was very quick. We two also went through separate treatments [with anitibiotic - Tab. Augmentin 650 (wife), Tab. Zifi 200 (myself)] and on recovery now.
But for me the mucus comes out through coughing in the early morning (once or twice for last two days and mostly 15-30 mins after I inhale steam). However, with Tab. Hetrazan and continued Tab. Zifi, I am feeling better today.

My worry is, can the mucus stay inside the lungs without making us cough?
If yes, how to identify it? Some tests?
My son is not coughing that much now. Does it mean, there is no mucus left?

2) If it is viral illness (the doctor prescribing Syp. Azithral was also of the opinion that it is viral --but the other doctor who prescribed Banocide said it is bacterial -- which confused me), how is it that the my son started feeling better only after taking Syp. Azithral? Also, we other two in the family also went through antibiotic treatments. I remember even last year when my son went through a viral illness, doctors treated with antibiotics.

3) My son likes citrus fruits, milk, biscuits. Is it okay to give him those when he is recovering from coughing?

4) As little bit of coughing is still there, will inhaling steam (plain steam - no medicine) for 5-10 minutes help him recover faster? Out of curiosity -- how steam helps fight respiratory infection?

5) In the long term, is it safe to inhale steam everyday for 5-10 minutes? I mean, can this act as preventive measure in the long term? For kids? For adults?

6) What general preventive measures do you suggest to avoid recurrence of respiratory infections (be it viral or bacterial)?

I am asking lot of questions. But I hope all of these are relevant to the illness under discussion.

Suggestions on viral infection

Detailed Answer:
Hi...

1. The fact that you both parents got it...itself is a proof that it is viral illness and not bacterial. Viral infection only spreads like that not bacterial.

2. Mucus is normal in Lungs. No tests to prove it. It will get absorbed by itself.

3. Even if you all three have not used antibiotics it would have gotten better at same time. As you have already taken antibiotics you feel that it's because of then you got better.

4. Citrus fruits are rich source of vitamin C which is good for general resistance and allergy. You can give them to your son.

5. There is no scientific evidence to suggest that steam inhalation will help fight or prevent respiratory infections. I do not suggest it.

6. General preventive measures are good proteinacious diet, fresh air free of pollution, avoiding crowded areas while flu or viral respiratory illnesses are breaking out in the community.

Please get back to me on his progress.

Hello Doctor,
My son is feeling better today in terms of activity. He is playing and eating well.

Present condition:
1) But cold is still there and mucus -- yellowish and thicker than earlier -- flows through the nose every now and then. Cold seem to have increased compared to last few days.

2) Little bit of coughing is also present but no mucus thrown out through mouth, coughing is not that deep.

3) Fever is absent but sweating continues to occur.

4) Not sure what are they but I have noticed few pimples - very small - on his face, hands, legs, torso, etc. 2-3 in count everywhere, so total 8-10 as of now.

5) His motion is regular today. He has urinated 3-4 times with less than regular quantity but similar to last 3-4 days.

Medicine given today:
We have given him Syp Atarax 2.5 ml twice today (as per the doctor's prescription) as the cold does not seem to be subsiding. No other medicine.

Questions:
1) Should we continue with Syp. Atarax 2.5 ml twice everyday? If yes, how long this should be taken? In other words, how long it will take to treat the cold completely?

2) Should we see the doctor for checking what those pimples are? I am not sure if they are caused by sweating. Is it okay to see if they change over next few days and then visit doctor if required? What signs to look for here?

1. Kindly upload the photos of the rash or pimples that you have noticed on your kid.

2. You can continue atarax for 3-5 days depending on severity of cold.

3. It could be heat rash or viral exanthema or hand foot mouth disease or insect bite allergy. I need to see the images before deciding on what it is.

Hi....I have seen the images just now.

The pimple looks like a small water filled papule. But it is too early to comment and the pictures are also slightly blurred. With this in mind I put forth these possibilities -

1. Chickenpox starting stage.
2. Viral exanthema.
3. Insect bite allergy.

These are not due to sweating.

I suggest you consult your pediatrician first thing in the morning.

If there's high grade fever or if the papules are spreading fast I suggest you approach the nearest emergency room as soon as possible.
