Question: looking for dosage of injectable lidocaine as anesthetic for wound care in 300 pound man. Wound in 5 cm. deep and narrow. Looking to use silver nitrate to help widen neck of wound to keep area open for irrigations. I'm an RN, and would like to confirm dosage.

Dear Ms

Use of injectable local anaesthetic is not recommended in care of wound especially if there is local infection. Simple packing of the wound with gauge piece soaked in antibacterial agent will help keep the area open. Even if insist and want to use Lidocaine for local injection , the maximum dose it is 4.5mg/kg ( 1% Lidocaine solution )of body weight ( kindly use only Plain Lidocaine)> it would be advisable to dilute it 1:1 and use it diluted. volume of 10 ml would suffice

Thank you. I agree it shouldn't be used as an irrigation solution. Just wanted to numb the skin before dilating the opening and using the silver nitrate applicator to keep the exterior area open so we can continue with the saline irrigations. The whole is becoming too small resulting in difficulty inserting the iodoform packing strip as ordered by MD. As previously mentioned, the infection has been resolved. No growth in the last culture. Use of lidocaine is for a one time usage only for the procedure.

Lidocaine jelly can be used

Detailed Answer:
Dear Ms

Instead of using the Injectable Lidocaine , Lidocaine jelly or Ointment can be used and applied for 30 minutes prior to doing the application. I would not recommend injecting lidocaine locally around the wound.
