Brief Answer:
Not a good situation

Detailed Answer:
Hello

After going through the medical details provided by you of a 82 yrs old person who have been a heavy smoker for a long time and have a heart function of 20% is not in a good medical condition.

Normally heart function is around 60% but in his case it's only 20% which is categorised as severe cardiac dysfunction that too with leakage in valves do not have a good prognosis.

However we should wait till his cardiac XXXXXXX is done as it will clear the picture that how much blockage is there in his coronary arteries. Treatment plan will depends on the outcome of cardiac XXXXXXX

As smoking promotes atherosclerosis which leads to plaque development inside the coronaries.

I hope this information will answer your query. If you have any other doubts you can ask me back or else you can close the discussion and kindly rate the answer.

I wish him good health

Kind Regards
