Brief Answer:
Decrease INR or clinical symptom should prompt to look for emboli.

Detailed Answer:
Hello,
Thanks for posting your query on Health Care Magic.

What is the status of INR after starting Xarelto?
The INR needs to be kept up to 2.5 and should be monitored consistently .
Any sudden decrease in INR needs to be followed with care.

A search for recurrent clot needs to be done if there is any change in INR values.
In addition, clinical symptom like pain or swelling in lower limbs or any symptom pertaining to chest like chest pain, dyspnea should also be reviewed with caution.
INR can be checked once in a month ideally.

In addition, D-dimer and fibrin degradation product are two investigations which will help to detect any embolic phenomenon in body.
