Question: What is the recommended laxative protocol for serious constpation and cramps due to hemmroids and anal fissure. An X-ray showed an abundance of stool however cat scan and blood test ok. I have been having serious cramps and constipation for a month and sporadically use miralox and stroll softeners, plus pills to move bowels. Need a routine other than drinking water that would be effective.

I appreciate your concern

Ondansetron can lead to constipation. It should be excluded to observe for improvement in the symptoms. Better alternative would be itopride or metoclopramide.

For the constipation associated with anal fissure and hemorrhoids, Cremaffin Plus Syrup containing Liquid Paraffin, Magnesium Hydroxide and Sodium Picosulfate should be used 2 full table spoons thrice a day. In addition, a warm glass of milk with 15 ml olive oil mixed with 2 table spoons of ispaghula husk should be taken every night before going to sleep. This will adequately relieve the constipation.

The anal fissure and hemorrhoids should be properly managed. A general surgeon should examine the area. Physical examination including a digital rectal exam would be required to assess the severity and treatment can be advised accordingly. For example , if the fissure is not healing with maximal medical therapy like sphincter relaxing ointment e.g nitrates (GTN ointment) and posterisone forte then surgical treatment in the form of a lateral internal sphincterotomy should be advised, in consultation with your doctor.
