Question: My Brother is diagnosed with HIGH GRADE "B" CELL NON HODGKINS LYMPHOMA OF PANCREAS with DUODENAL INFILTRATION. I would like to know the
1. treatment we are taking is correct or not and how serious is the situation.
2. Currently we are showing to a medical oncologist, Is it fine or do we need to visit hemato oncologist .
3. Recommendations for any hospital in south XXXXXXX

Cd 20 negative b cell lymphoma is a rare entity.
Biopsy should be re confirmed by another pathologist.
If it is again confirmed that it is b cell lymphoma, then same treatment can be continued, as treatment is alright.
Alk negative cd 20 lymphoma is aggressive and prognosis is guarded.
Ttreatimg doctor need not to be changed.

Hello dear. Lymphoma which are alk negative and cd20 negative have poor prognosis. Although they respond to chemotherapy well, they have high chances of relapses.

3) How many Chemo's need to be done ?
4) We have not done the PET scan, Do you suggest PET scan?
5) We have given the second chemo last week, After which he got fever, cold, cough and he is feeling too much fatigue. Is these expected? Feeling very much feared.

Hello dear. Relapse cant be prevented if it is bound to happen despite optimum treatment.
Stage of the disease cant be determined as only ct abdomen report is available. However, it is not going to change the line of treatment.
6 cycles of chemotherapy needs to be done.
You can get a pet scan done just prior to next cycle of therapy to know the stage of disease.
Fever and cough are signs of infection. That may occur during chemotherapy as patients of cancer have low immunity.
Prophylactic antibacterial antiviral and antifungal drugs can be added to prevent infection. You can discuss these with your doctor.
