Question: Dear Dr XXXXXXX I hope that this question is not too vague. Are treatments options for cardiac failure involving pulmonary hypertension exactly the same as treatment options for cardiac failure that do not involve pulmonary hypertension?

No, definitely it is not a vague question.
The fundamental thing to be understood is that if cardiac failure has occurred as a result of pulmonary hypertension or pulmonary hypertension is a result of cardiac failure. Both scenarios are different and have different treatment approach.
However, for patients where pulmonary hypertension is not the cause of cardiac failure, treatment options are similar for those with or without pulmonary hypertension. Only difference will be that prognostically those with pulmonary hypertension will fare worse than those without it.
Let me explain, in certain situations heart failure occur secondary to pulmonary hypertension like chronic bronchitis, or idiopathic pulmonary hypertension. All these patients will have pulmonary hypertension and heart failure but basic treatment will be target different things in different conditions. However in those with cardiac failure with secondary pulmonary hypertension , we will aim to treat heart failure and pulmonary hypertension will get corrected on its own.
