Brief Answer:
Can be due to stem loosening.

Detailed Answer:
Hello,
Thanks for posting your query.
Inability to walk on the replaced hip after fifteen years of replacement can be due to stem loosening. This pain often radiates to the knee. Loosening of previous generation actabular components increases markedly after 12-15 years of function. Obesity and strenuous activities can contribute to failure of the acetabular component. Mild trauma can fracture the femur around a prosthesis stem, especially if the bone is osteoporotic.
You need to consult an orthopedician for a detailed evaluation. MRI scan can help in proper diagnosis.
I hope this answers your query.
