What Medical Check Up Is Advisable When Periods Gets Delayed By A Week While Trying To Conceive?

Question: Hello Doctor, This regarding my wife who is 30yrs old (weighs 80kg). We are a little uncertain/certain of her chances of being pregnant. We've been trying to conceive for the past one year. Although it has been over an year that we've stopped using protection, however it was in this month during her 10th day of ovulation where we were told that chances of conceiving is higher, we tried intercourse. The concern is her menstrual cycle is delayed by seven days and above. Can you suggest what medical check up we should adhere to? We're planning to go and visit an OB-GY doctor, but were wondering how could we prepare better and make the most from the consultation. What should be the good questions to be asked, etc?

2. History of regular menstrual cycle - yes
3. Has she started on preconception vitamins supplements - None
4. Any other tests or investigation - None
5. Is there any problem with sexual activity - No

Hi,

As per your history, I understand her menstrual cycle is delayed by 7 days. In view of her history of regular cycles, I will suggest you to get morning's first sample of urine tested for pregnancy. In case of any doubt blood beta HCG test and USG will be of help.

As you have mentioned about her height and weight, I would like to suggest you something about it. Please do not take it personally or as criticism. I will suggest that she has body mass index of 28, as against it should have been 23.

I usually suggest to my patients to follow my advice as follows-
- Please opt for a healthy diet and regular exercise regimen.
- Include more portions of fruits and vegetables and salads in daily diet.
- Drink plenty of water in the day.
- Avoid refined sugars and deep fried foods and bakery products in the diet.
- Please take Iron, Calcium, protein and vitamin B12 and Folic acid supplementation on a regular basis.
All this will be of help to prevent various complications during conception, pregnancy and childbirth.

Secondly, as you are planning to conceive, I would like you to know few things.

Before going ahead with any other kind of treatment, I will suggest you to get following tests done.
1. Your semen analysis to know about the quality and quantity of sperms.
2. Your wife's USG , to know about the structural normalcy of the uterus and ovaries.
3. Your wife's post menstrual HSG done-Hysterosalpingography, to know about the patency of fallopian tubes.

When these reports are within normal limits, almost 90% of the couples get pregnant within a year.

As you might be aware that in a woman with regular cycle, day 10 to 20 of the cycle is the most fertile period of the cycle.
If you try to have unprotected sex activity in this period, at least on alternate days,then you stand good chance to get pregnant.

Thank you very much doctor for your advice. We got the urine test done today morning, and the result came as positive. I still can't believe this is happening, we are estatic. But I'm still not sure, whether to confirm or not if my wife is now conceiving. Now as recommended, we shall try to get the other tests you mentioned. Have scheduled for an appointment on Thursday. I will suggest you to get her USG done to know about the exact gestational age and viability of the pregnancy.
