3 Dextroamphetamine 10 mg. 2 a.m./1 noon - to control severe Anorexia Bulemia I've struggled with for 49 years
1 Paroxetine 20 mg. a.m.
2 Levocetirizine 5 mg. a.m/p.m.
1 Omeprazole 40 mg. a.m.
3 Tizanidine 4mg. a.m.
1 Donepezil 26 mg. a.m.
6 Gabapentin 100 mg. a.m./noon/p.m. [200 mg. at time]
1 Meloxicam 15 mg. a.m.
1 Estradiol 1 mg. a.m.
4 Oxycodone 15 mg. a.m./noon/5:00 pm./ bed time
2 Alprazolam 1 mg. p.m.
1 Trazodone 150 mg. p.m.
1 Lorazopam 1 mg. as needed only for severe panic attacks induced by chronic pain
I Multi-vitamin for Seniors a.m.
1 B 12 2500 mcg.a.m.
1 D3 2000 IU.a.m.
1 Potassium Gluconate 550 mg. a,m,
I've had 18 surgeries, the last 5 were for shoulder, neck and back injuries. Four were on the left shoulder and one was on the right shoulder. The 4 left surgeries occurred within a 2 year period due to multiple injuries. The last left surgery was a total reverse shoulder replacement surgery which has been very painful but 3 surgeons believe is well fixed. Unfortunately the top on the shoulder bone has cracked due to continuous falls and the apparatus places too much pressure on a nerve causing much pain. Nothing more can be done without immobilizing my shoulder.
I fear that my meds are preventing the donepezil from working as it should.

I have gone through your medications extensively and would like you to consider the following points.
There is an issue of polypharmacy and probability of multiple interactions exist.
The DONEPEZIL levels are decreased by the sedatives like alprazolam.
Donepezil levels are also decreased by concomitant trazodone use.
though you didn't mention any antibiotics, Antibiotics like Clarithromycin can raise the levels of donepezil significantly.
Trazodone, alprazolam and lorazepam doses can be lowered to achieve maximum Donepezil affect.
All such interactions would require monitoring.
I would advise you to undergo the following investigations again.
Serum Electrolytes, Renal Function Tests, Liver Function Tests, Urine R/E, FBS.
Such a large number of drugs are bound to cause some compromise on one or more of the above investigations. If so , the number of drugs would definitely need to be reduced depending on whether they are nephrotoxic or hepatotoxic.
Alprazolam and lorazepam have similar indications, you may use only one of them.
Pain medications include Gabapentin, meloxicam, tizanidine, oxycodone. Depending upon the results of the above investigations dose adjustment can be done to see if you could tolerate lower doses or skipping a drug in between.
